Linezolid is a synthetic antibiotic belonging to the oxazolidinone class, effective against Gram-positive aerobic bacteria, some Gram-positive anaerobic bacteria, and certain Gram-negative bacteria. It works by inhibiting bacterial protein synthesis through a unique mechanism: it binds to the 23S ribosomal RNA of the 50S bacterial ribosomal subunit, blocking the formation of the 70S initiation complex necessary for bacterial protein production.
Bacteriostatic effect: against enterococci and staphylococci.

✅ Description:
Lizid is used to treat the following infections:
- Nosocomial pneumonia (hospital-acquired pneumonia)
- Community-acquired pneumonia
- Complicated skin and skin structure infections, including diabetic foot infections without bone involvement (osteomyelitis)
- Uncomplicated skin and skin structure infections
- Vancomycin-resistant Enterococcus faecium infections
- Limitations: Lizid is not effective against Gram-negative infections. The safety and effectiveness of Lizid for treatments longer than 28 days have not been studied in controlled clinical trials.

Uncomplicated Skin and Skin Structure Infections:
400 mg orally every 12 hours for 10–14 days.
Vancomycin-Resistant Enterococcus faecium Infections:
600 mg orally every 12 hours for 14–28 days.
Methicillin-Resistant Staphylococcus aureus (MRSA) Infections:
600 mg orally every 12 hours for 7–21 days.
Community-Acquired Pneumonia (CAP), Nosocomial Pneumonia, and Complicated Skin Infections:
600 mg orally every 12 hours for 10–14 days.
Elderly Patients:
No dosage adjustment needed.
Hepatic Impairment (Mild to Moderate):
No dosage adjustment needed.
Renal Impairment Dosage
No dosage adjustment is needed for patients with renal impairment.

Pneumonia, Complicated Skin Infections, Vancomycin-Resistant Enterococcus faecium:
Birth–11 years: 30 mg/kg/day divided every 8 hours (q8h).
>11 years: 1,200 mg/day, divided every 12 hours (q12h).
Uncomplicated Skin Infections:
Birth–5 years: 30 mg/kg/day q8h.
5–11 years: 20 mg/kg/day q12h.
>11–18 years: 1,200 mg/day q12h.
✔️ Drug Interactions
- Monoamine Oxidase Inhibitor (MAOI) Activity: Lizid is a nonselective, reversible MAOI. It may interact with adrenergic and serotonergic agents, enhancing the pressor response to indirect sympathomimetic agents or vasopressors.
- Serotonin Syndrome Risk: Avoid use with serotonin reuptake inhibitors, tricyclic antidepressants, serotonin receptor agonists (triptans), meperidine, or buspirone unless closely monitored.
- Tyramine Interaction: Avoid large amounts of foods/beverages high in tyramine (e.g., aged cheeses, cured meats).
✔️ Lizid must not be used in patients who:
- Are allergic to linezolid or its components.
- Are taking MAO inhibitors or have taken them in the last 2 weeks (e.g., Phenelzine, Isocarboxazid).
- Have uncontrolled hypertension, pheochromocytoma, thyrotoxicosis, or carcinoid syndrome.
- Are taking adrenergic agents (e.g., Pseudoephedrine), vasopressors (e.g., epinephrine, norepinephrine), dopaminergic agents (e.g., dopamine, dobutamine), or serotonin-modifying drugs as mentioned above.
✔️ Pregnancy & Lactation
- Pregnancy: Limited data show no major birth defect risks, but Lizid should only be used if the potential benefit justifies the risk.
- Lactation: Linezolid is present in breast milk. Consider breastfeeding benefits against potential risks. Monitor infants for diarrhea or vomiting.
✔️ Precautions & Warnings
- Blood Monitoring: Weekly complete blood counts are essential, especially if treatment exceeds two weeks or in patients with bone marrow suppression.
- Visual Issues: Prompt ophthalmic evaluation for vision changes (e.g., acuity, color vision, blurriness).
- CDAD (Clostridium difficile-associated diarrhea): Can occur up to two months post-treatment. Discontinue antibiotics not effective against C. difficile if CDAD is suspected.
- Hypertension Risk: Monitor blood pressure closely; avoid it in patients with uncontrolled hypertension or related conditions.
- Metabolic Effects: If nausea, vomiting, or acidosis occurs, seek immediate medical attention. Adjust insulin or hypoglycemic doses if hypoglycemia occurs.
