Azithromycin is acid-stable and effectively absorbed when taken orally, especially on an empty stomach. It reaches peak concentration in 2.1 to 3.2 hours. The drug accumulates in phagocytes and is released at infection sites. It has a long half-life, allowing prolonged action even after a single dose. Excretion occurs mainly through bile, with a small percentage eliminated via urine.

✔️ Mechanism of Action
Azithromycin binds to the 50S ribosomal subunit of bacteria, disrupting protein synthesis. It is effective against:
- Gram-positive bacteria: Staphylococcus aureus, Streptococcus pneumoniae, Streptococcus pyogenes.
- Gram-negative bacteria: Haemophilus influenzae, Neisseria gonorrhoeae.
- Other pathogens: Chlamydia pneumoniae, Mycoplasma pneumoniae, Legionella pneumophila.

- Adults:
- 500 mg daily for 3 days, or
- 500 mg on day 1, then 250 mg daily for 4 more days.
- For Chlamydia: 1 g as a single dose, or 500 mg on day 1, followed by 250 mg daily for 2 days.
- Children (over 6 months):
- 10 mg/kg once daily for 3 days.
- Adjusted dosages based on body weight:
- 15-25 kg: 200 mg/day
- 26-35 kg: 300 mg/day
- 36-45 kg: 400 mg/day
- For Typhoid Fever: 500 mg daily for 7-10 days.

- Antacids: Take azithromycin at least 1 hour before or 2 hours after antacids.
- Carbamazepine, Methylprednisolone: No significant interaction.
- Cyclosporin, Digoxin: May require dosage adjustments.
- Ergot Derivatives: Avoid co-administration due to risk of ergotism.
- Warfarin: No significant effect, but monitor prothrombin time.
- Theophylline: No known interaction, but monitoring is advised.
