Cefuroxime is a broad-spectrum, bactericidal cephalosporin antibiotic. It works by binding to bacterial penicillin-binding proteins and inhibiting the final stages of bacterial cell-wall synthesis, ultimately causing bacterial cell death.

Cefuroxime is relatively stable against many bacterial beta-lactamases and may therefore remain active against a number of ampicillin- or amoxicillin-resistant strains. Its effectiveness depends on the susceptibility of the infecting organism.

Axim (Cefuroxime) is a second-generation cephalosporin antibiotic indicated for the treatment of infections caused by susceptible bacteria, including:

  • Pharyngitis and tonsillitis caused by Streptococcus pyogenes.
  • Acute bacterial otitis media caused by Streptococcus pneumoniae, Haemophilus influenzae, Moraxella catarrhalis (including beta-lactamase-producing strains), or Streptococcus pyogenes.
  • Acute bacterial maxillary sinusitis caused by susceptible Streptococcus pneumoniae or Haemophilus influenzae.
  • Lower respiratory tract infections, including pneumonia, caused by susceptible organisms such as Streptococcus pneumoniae, Haemophilus influenzae, Klebsiella spp., Staphylococcus aureus, Streptococcus pyogenes, and E. coli.
  • Acute bacterial exacerbations of chronic bronchitis and secondary bacterial infections of acute bronchitis.
  • Skin and skin-structure infections caused by susceptible Staphylococcus aureus or Streptococcus pyogenes.
  • Urinary tract infections caused by susceptible E. coli or Klebsiella pneumoniae.
  • Bone and joint infections caused by susceptible Staphylococcus aureus.
  • Gonorrhoea caused by susceptible Neisseria gonorrhoeae.
  • Early Lyme disease (erythema migrans) caused by Borrelia burgdorferi.

✔️ Parenteral Administration

Injectable cefuroxime should be administered by a qualified healthcare professional.

Adults:

  • Usual dose: 750 mg three times daily by IM or IV injection.
  • For severe infections: up to 1.5 g three times daily by IV injection may be used.
  • In some severe infections, the frequency may be increased to four times daily under specialist supervision.

Children above 3 months:

  • 30–100 mg/kg/day in 3 or 4 equally divided doses.
  • A dose of 60 mg/kg/day is appropriate for many infections.

Neonates:

  • 30–100 mg/kg/day in 2 or 3 equally divided doses.

Surgical prophylaxis:

  • 1.5 g IV at induction of anaesthesia. Additional doses may be given according to the surgical procedure and clinical risk.

Pneumonia:

  • 1.5 g IV twice daily for 2–3 days, followed by appropriate oral therapy when clinically indicated.

Acute exacerbation of chronic bronchitis:

  • 750 mg IM or IV twice daily for 2–3 days, followed by 500 mg orally twice daily for 5–10 days when clinically appropriate.

Gonorrhoea:

  • 1.5 g as a single IM dose may be used according to the prescribed regimen.

Meningitis:

  • Adults: 3 g IV three times daily.
  • Children above 3 months: 200–240 mg/kg/day IV in 3 or 4 divided doses, subsequently reduced according to clinical improvement.
  • Neonates: dosing should be individualized by the treating physician.

Bone and joint infections:

  • Adults: 1.5 g IV four times daily.
  • Children above 3 months: up to 150 mg/kg/day in divided doses, without exceeding the recommended adult maximum.

Adults and adolescents 13 years and older:

  • Pharyngitis/Tonsillitis: 250 mg twice daily for 5–10 days.
  • Acute bacterial maxillary sinusitis: 250 mg twice daily for 10 days.
  • Acute exacerbation of chronic bronchitis: 250–500 mg twice daily for 10 days.
  • Secondary bacterial infection of acute bronchitis: 250–500 mg twice daily for 5–10 days.
  • Uncomplicated skin and skin-structure infections: 250–500 mg twice daily for 10 days.
  • Uncomplicated urinary tract infections: 250 mg twice daily for 7–10 days.
  • Uncomplicated gonorrhoea: 1,000 mg as a single dose.
  • Community-acquired pneumonia: 250–500 mg twice daily for 5–10 days.
  • MDR typhoid fever: 500 mg twice daily for 10–14 days.
  • Early Lyme disease: 500 mg twice daily for 20 days.

Children 3 months to 12 years:

  • Pharyngitis/Tonsillitis: 20 mg/kg/day in two divided doses for 5–10 days.
  • Acute otitis media: 30 mg/kg/day in two divided doses for 10 days.
  • Acute bacterial maxillary sinusitis: 30 mg/kg/day in two divided doses for 10 days.
  • Impetigo: 30 mg/kg/day in two divided doses for 10 days.

The exact dose and duration should be determined by the treating physician based on the infection, age, body weight, kidney function, and clinical response.

✔️ Cefuroxime is generally well tolerated. Possible side effects include:
  • Nausea, vomiting, diarrhoea, or abdominal discomfort
  • Skin rash or itching
  • Headache or dizziness
  • Injection-site pain or irritation with parenteral administration
  • Temporary changes in liver enzymes
  • Candida overgrowth with prolonged antibiotic use
