✔️ Dosage and Administration | Ramipril 2.5

Hypertension

Patients not taking diuretics

• Initial dose: 2.5 mg once daily at bedtime.
• Maintenance dose: 2.5–5 mg daily.
• Maximum dose: 10 mg daily.

Patients taking diuretics

• Initial dose: 1.25 mg once daily at bedtime.

Congestive Heart Failure

• Initial dose: 1.25 mg once daily.
• Doses above 2.5 mg may be divided into two daily doses.
• Maximum dose: 10 mg daily.

Post-Myocardial Infarction

• Initial dose: 2.5 mg twice daily.
• May be increased to 5 mg twice daily after two days if tolerated.
• Treatment generally begins 3–10 days after the heart attack.
• Maintenance dose: 2.5–5 mg twice daily.

Prevention of Cardiovascular Events and Stroke

• Initial dose: 2.5 mg once daily.
• May be increased to 5 mg daily after one week if tolerated.
• Maintenance dose: 10 mg once daily after an additional three weeks.

Hepatic Impairment

• Maximum recommended dose: 2.5 mg daily.

✔️ Pregnancy & Lactation

Pregnancy

• Ramipril is contraindicated during pregnancy.
• ACE inhibitors can cause serious injury or death to the developing fetus, especially during the second and third trimesters.

Breastfeeding

• Ramipril may pass into breast milk.
• Breastfeeding is generally not recommended during treatment.

✔️ Directions for Use: Ramipril 2.5

• May be taken with or without food.
• Swallow the tablet whole.
• Do not crush, chew, or break the tablet.
• Take it at the same time every day to achieve the best results.

✔️ Acecard 2.5 should not be used in patients with:

• Known hypersensitivity to ramipril or other ACE inhibitors
• History of angioedema (hereditary, idiopathic, or ACE inhibitor-related)
• Bilateral renal artery stenosis
• Unilateral renal artery stenosis in a solitary functioning kidney
• Significant aortic stenosis or outflow tract obstruction
• Pregnancy
• Breastfeeding (unless specifically advised by a physician)

✔️ Words of Advice:

• Take the medicine at the same time every day.
• Standing up slowly can help reduce dizziness when starting treatment.
• Inform your doctor if you develop a persistent dry cough or throat irritation.
• Avoid potassium supplements and excessive intake of potassium-rich foods unless advised otherwise.
• Continue taking the medication even if you feel well.
• Do not stop treatment without consulting your doctor.

✔️ Ramipril may interact with:

• Diuretics and other antihypertensive medications, increasing the risk of excessive blood pressure reduction.
• NSAIDs (such as ibuprofen and naproxen), which may reduce the antihypertensive effect and worsen kidney function.
• Lithium, increasing the risk of lithium toxicity.
• Potassium-sparing diuretics (e.g., spironolactone, amiloride), potassium supplements, and potassium-containing salt substitutes, increasing the risk of hyperkalemia.
• Aliskiren, especially in diabetic patients or those with kidney impairment, which may increase the risk of hypotension, kidney injury, and elevated potassium levels.

✔️ Use with caution in patients with:

• Kidney impairment
• Low blood volume (hypovolemia)
• Hyperkalemia
• Heart valve disease
• Severe or resistant hypertension
• Peripheral vascular disease
• Generalized atherosclerosis
• Elderly age

Patients should:

• Avoid activities requiring mental alertness until they know how the medicine affects them.
• Correct dehydration or salt depletion before starting treatment.
• Undergo regular monitoring of blood pressure, kidney function, and potassium levels.

Renal Impairment

Dose adjustment may be required:

Kidney function should be monitored closely during the first few weeks of treatment and periodically thereafter.
