Esomeprazole, a proton pump inhibitor, works by inhibiting the H+/K+-ATPase in the gastric parietal cell, thereby suppressing gastric acid secretion. It is the S-isomer of omeprazole and the first single optical isomer of a proton pump inhibitor, offering improved acid control compared to racemic proton pump inhibitors.

After oral administration, esomeprazole is absorbed with peak plasma levels occurring around 1.5 hours. Increasing the dose leads to a proportional increase in peak plasma levels, with systemic bioavailability approximately 90% after repeated once-daily dosing. However, food intake decreases the absorption of esomeprazole, resulting in a 33-53% decrease in plasma levels when taken with food. Therefore, it is recommended to take esomeprazole at least one hour before meals.

Esomeprazole is extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily CYP2C19 and to a lesser extent CYP3A4. The metabolites lack anti-secretory activity. Only a small fraction of the parent drug is excreted unchanged in the urine, with approximately 80% excreted as inactive metabolites in urine and feces combined.

In combination therapy with antimicrobials like clarithromycin and amoxicillin, esomeprazole's pharmacokinetic parameters may be altered, with increases in steady-state AUC and Cmax. However, these changes are not considered clinically significant.

✔️ Dosage & Administration:
  • To heal erosive esophagitis, Xelpro is typically taken orally at either 20 mg or 40 mg once daily for 4 to 8 weeks. If healing hasn't occurred after this period, an additional 4 to 8 weeks of treatment may be considered. For maintenance therapy to sustain healing, a dose of 20 mg once daily is recommended, although clinical studies did not extend beyond 6 months.
  • For symptomatic gastroesophageal reflux disease (GERD), a dosage of 20 mg once daily is typically prescribed for 4 weeks. If symptoms persist after this period, an additional 4 weeks of treatment may be warranted.
  • Helicobacter pylori eradication typically involves triple therapy, consisting of esomeprazole 40 mg once daily, amoxicillin 1000 mg twice daily, and clarithromycin 500 mg twice daily, all taken for 10 days.
  • For Zollinger-Ellison Syndrome, the recommended dosage of Xelpro ranges from 20 mg to 80 mg once daily, with individual adjustments as needed based on clinical response.
  • In cases of acid-related dyspepsia, Xelpro is usually taken at a dose of 20 mg to 40 mg once daily for 2 to 4 weeks, depending on the patient's response.
  • For duodenal ulcers, a dosage of 20 mg once daily for 2 to 4 weeks is typically prescribed, while gastric ulcers may require a higher dose of 20 mg to 40 mg once daily for a duration of 4 to 8 weeks.

Esomeprazole should be swallowed whole and taken one hour before a meal.

Xelpro may interfere with drugs metabolized by CYP2C19 and affect the absorption of drugs sensitive to gastric pH. It doesn't significantly interact with many common medications, but caution is needed with diazepam and drugs affected by gastric pH.
