Pharmacology

Esomeprazole is a proton pump inhibitor that inhibits the H+/K+-ATPase in the gastric parietal cell, suppressing gastric acid output. The first single optical isomer of a proton pump inhibitor, esomeprazole (S-isomer of omeprazole), provides superior acid control than racemic proton pump inhibitors.

Esomeprazole capsules contain an enteric-coated pellet version of esomeprazole magnesium for improved absorption. Peak plasma levels (Cmax) occur roughly 1.5 hours after oral dosing (Tmax). When the dose is increased, the Cmax increases correspondingly, and the area under the plasma concentration-time curve (AUC) increases thrice from 20 to 40 mg. The systemic bioavailability with repeated once-daily doses is around 90%, compared to 64% after a single dose. When compared to fasting conditions, the AUC following a single dosage of esomeprazole is reduced by 33-53 percent after food ingestion. At least one hour before meals, esomeprazole should be consumed.

Esomeprazole binds to plasma proteins 97 percent of the time. Over a concentration range of 2 20 mmol/L, plasma protein binding remains constant. In healthy volunteers, the apparent volume of distribution at a steady state is around 16 L.

Esomeprazole is extensively processed by the cytochrome P450 (CYP) enzyme system in the liver. Esomeprazole's metabolites have no anti-secretory properties. The CYP2C19 isoenzyme, which creates the hydroxy and desmethyl metabolites, is responsible for the majority of esomeprazole metabolism. The sulphone metabolite is formed by CYP3A4, which is responsible for the remaining proportion.

Dosage & Administrations of Sergel Mups 20mg Tablet

The recommended dosage of Esomeprazole Mups tablet varies depending on the condition and age of the patient. 

• For Adults with Erosive Esophagitis, the recommended dosage is 40 mg once daily for 4 weeks, and for children and adolescents between 12-18 years, the dosage is the same.
• For maintaining the healing of erosive esophagitis in adults, the recommended dosage is 20 mg once daily, and for children and adolescents between 12-18 years, the dosage is the same.
• For reducing the risk of NSAID-associated gastric ulcers in adults, the recommended dosage is 20 mg once daily for 4-8 weeks.
• For H. pylori eradication, adults should take 20 mg twice daily for 7 days, and children and adolescents between 12-18 years should take the same dosage.
• For Zollinger-Ellison syndrome and idiopathic hypersecretion, the recommended dosage for adults is 40-80 mg twice daily
• For children aged 1-11 years, the recommended dosage of Esomeprazole Mups tablet for erosive esophagitis depends on their weight.
• Children weighing less than 20 kg should take 10 mg once daily for 8 weeks, while those weighing 20 kg or more should take either 10 mg or 20 mg once daily for the same period.
• The recommended dosage for maintaining the healing of erosive esophagitis in children aged 1-11 years is 10 mg once daily.

Esomeprazole Mups tablet is not approved for use in children younger than 1 year.

Interaction

Iron supplements, blood thinners (clopidogrel, warfarin), antifungals (ketoconazole, voriconazole, itraconazole, posaconazole), antiviral medications (nelfinavir), anti-HIV medications (atazanavir, nelfinavir, rilpivirine), antibiotics (ampicillin, rifampicin), heart medications (digoxin), and anticancer medications (nelfina (methotrexate). If you use these medications, tell your doctor.

The antidepressant St. John's wort plant interacts with Sergel Mups 20mg Tablet. The functioning of Sergel Mups 20mg Tablet may be impacted by this plant. Therefore, avoid taking Sergel Mups 20mg Tablet with it.

Patients with acute interstitial nephritis, atrophic gastritis (inflamed stomach lining), magnesium and vitamin B12 insufficiency, and diarrhea caused by Clostridium difficile should not be administered Sergel Mups 20mg Tablet. (inflamed cells of the kidney).

Contraindications

In patients who have a history of hypersensitivity to any of the formulations, esomeprazole is not recommended.

Pregnancy & Lactation

In pregnant women, there are no sufficient and well-controlled trials. No teratogenic effects have been found in animal investigations. Esomeprazole excretion in milk has not been studied. If the use of esomeprazole is thought necessary, breastfeeding should be terminated.

Precautions & Warnings

If you are allergic to Sergel Mups 20mg Tablet or proton pump inhibitors, suffer from stomach cancer, liver illness, low magnesium levels (osteoporosis), low vitamin B12 levels, are pregnant or trying to get pregnant, or are nursing, you should not use Sergel Mups 20mg Tablet. 
Interactions with iron supplements, ampicillin antibiotic, a warfarin blood thinner, ketoconazole antifungal, atazanavir, nelfinavir, anti-HIV medication, and anti-cancer medication (methotrexate). If you use these medications, tell your doctor. Long-term usage of Sergel Mups 20mg Tablet raises the risk of magnesium, Vitamin B12, and lupus erythematosus, an inflammatory disorder in which the immune system destroys its own tissues. Consuming Sergel Mups 20mg Tablet may conceal gastric cancer symptoms, therefore if you have any significant stomach discomfort or gastric bleeding (blood in your mucus or stools), call your doctor right once. 

Storage Conditions

Store in a dry area at a temperature of not more than 30°C. Light and dampness should be avoided. Keep out of children's reach.
