✔️ Common Uses of Esomeprazole

• Gastroesophageal reflux disease (GERD)
• Heartburn and acid-related dyspepsia
• Peptic ulcer disease
• Erosive esophagitis
• Gastric and duodenal ulcers
• Zollinger–Ellison syndrome
• Helicobacter pylori infection as part of combination therapy
• Prevention and treatment of NSAID-associated gastric ulcers

✔️ Side Effects

✔️ Dosage varies according to the condition:

• GERD without erosive esophagitis: 20 mg once daily for 4 weeks; an additional 4 weeks may be considered if symptoms persist.
• GERD with erosive esophagitis: 20–40 mg once daily for 4–8 weeks.
• Maintenance of healed erosive esophagitis: 20 mg once daily, up to 6 months.
• NSAID-associated gastric ulcer prevention: 20–40 mg once daily, up to 6 months.
• NSAID-induced gastric ulcer: 20 mg once daily for 4–8 weeks.
• Zollinger–Ellison syndrome: Initial dose of 80 mg/day in divided doses, with further adjustment according to response; higher doses may be required under specialist supervision.

✔️ Children

Dosing depends on age, weight, and indication. Safety and efficacy have not been established for some pediatric age groups, so children should receive esomeprazole only under medical supervision.

✔️ Pregnancy & Breastfeeding

There are limited data specifically regarding esomeprazole during pregnancy and breastfeeding. Treatment should be used when the potential benefits justify the potential risks and after consultation with a doctor.

✔️ Renal and Hepatic Impairment

• Kidney impairment: No dosage adjustment is generally required.
• Mild to moderate liver impairment: No dosage adjustment is generally required.
• Severe liver impairment: The dose should generally not exceed 20 mg/day.

✔️ Esomeprazole may interact with several medicines, including:

• Clopidogrel: may reduce its antiplatelet effect.
• Warfarin: may increase INR and prothrombin time.
• Tacrolimus: may increase tacrolimus concentrations.
• Methotrexate: may increase serum concentrations, particularly with high-dose therapy.
• Digoxin: may increase the risk of digoxin-related toxicity, especially when magnesium is low.
• Diazepam and other CYP2C19-metabolized medicines: their elimination may be affected.
• Ketoconazole, erlotinib, and iron salts: absorption may be reduced.
• Rilpivirine, atazanavir, and nelfinavir: gastric acid suppression may substantially reduce their concentrations and effects; concomitant use may be contraindicated or inappropriate.

✔️ Tell your doctor if you have:

• Severe liver disease
• Kidney disease
• Osteoporosis or a history of fractures
• Previous allergic reactions to PPIs
• Persistent or unexplained gastrointestinal symptoms

Seek medical attention for persistent watery diarrhea, severe abdominal pain, reduced urination, swelling, unusual fatigue, rash, or fever.

Long-term users may require monitoring of magnesium levels and other relevant laboratory parameters.
