Fungal infection is a skin disease in which a fungus attacks the tissue and causes infection. It may be contagious (spread from one person to another). The symptoms of a fungal infection include blisters, itching, redness, scaling, and swelling of the skin. Yeast infection is caused by a type of fungus called Candida and commonly occurs in moist and warm areas of the body. It is not contagious. It may cause infections in the mouth (oral thrush), vagina, nails, groin, or armpits.

Itraconazole is used to treat fungal infections of the mouth, throat, toenails, fingernails, or lungs. It works by inhibiting the production of ergosterol (a main component of the cell membranes of fungi). Thereby killing fungi and clearing the fungal infection.

Non-Systemic Fungal Infections

  • Vulvovaginal Candidiasis: 200 mg twice daily for 1 day.
  • Pityriasis Versicolor: 200 mg once daily for 7 days.
  • Tinea Corporis and Tinea Cruris: 100 mg once daily for 15 days or 200 mg once daily for 7 days.
  • Tinea Pedis and Tinea Manuum: 100 mg once daily for 30 days.
  • Oropharyngeal Candidiasis: 100 mg once daily for 15 days. In patients with AIDS or neutropenia, the dose may be increased to 200 mg once daily for an additional 15 days.
  • Onychomycosis (Nail Fungal Infection):
    • Continuous therapy: 200 mg once daily for 3 months.
    • Pulse therapy: 200 mg twice daily for 7 days, followed by a 21-day treatment-free interval.
    • Two pulse courses are generally recommended for fingernails and three courses for toenails.

Systemic Fungal Infections

  • Aspergillosis: 200 mg once daily for 2–5 months; increase to 200 mg twice daily for invasive or widespread infections.
  • Candidiasis: 100–200 mg once daily for 3 weeks to 7 months; increase to 200 mg twice daily for severe or disseminated disease.
  • Non-Meningeal Cryptococcosis: 200 mg once daily for 10 weeks.
  • Cryptococcal Meningitis: 200 mg twice daily for 2–6 months.
  • Histoplasmosis: 200 mg once or twice daily for approximately 8 months.
  • Maintenance Therapy in AIDS: 200 mg once daily until immune recovery.
  • Neutropenia Prophylaxis: 200 mg once daily until immune recovery.

Treatment duration and dosage for systemic infections should be individualized according to clinical response.

  • Vulvovaginal Candidiasis: 130 mg twice daily for 1 day.
  • Pityriasis Versicolor: 65 mg twice daily for 7 days.
  • Tinea Corporis and Tinea Cruris: 65 mg once daily for 15 days or 65 mg twice daily for 7 days.
  • Tinea Pedis and Tinea Manuum: 65 mg once daily for 30 days.
  • Oropharyngeal Candidiasis: 65 mg once daily for 15 days. In AIDS or neutropenic patients, the dose may be increased to 65 mg twice daily for 15 days.
  • Onychomycosis:
    • Continuous therapy: 65 mg twice daily for 3 months.
    • Pulse therapy: 130 mg twice daily for 7 days, repeated after a 21-day interval.
    • Two courses are generally recommended for fingernails and three courses for toenails.

Systemic Fungal Infections

  • Aspergillosis: 65 mg twice daily for 2–5 months; increase to 130 mg twice daily if required.
  • Candidiasis: 65–130 mg once daily for 3 weeks to 7 months; increase to 65 mg twice daily for severe infections.
  • Non-Meningeal Cryptococcosis: 65 mg twice daily for 10 weeks.
  • Cryptococcal Meningitis: 130 mg twice daily for 2–6 months.
  • Histoplasmosis: 130 mg once or twice daily for approximately 8 months.
  • Maintenance Therapy in AIDS: 65 mg twice daily until immune recovery.
  • Neutropenia Prophylaxis: 65 mg twice daily until immune recovery.

Dosage and treatment duration should be adjusted based on the patient's response to therapy.

Treatment of Non-Systemic and Systemic Fungal Infections

Applicable for conditions such as:

Children Older Than 4 Weeks to Less Than 12 Years

  • Usual dose: 1.25–2 mg/kg per dose twice daily (maximum initial dose 100 mg per dose).
  • For severe infections, the dose may be increased up to 5 mg/kg per dose twice daily (maximum initial dose 100 mg per dose).
  • Further dose adjustments should be guided by therapeutic drug monitoring.
  • Doses should be rounded to the nearest 50 mg for easier administration.

Children 12 Years and Older

  • Usual dose: 50–100 mg once daily.
  • In severe infections, the dose may be increased to 100 mg twice daily.

Prophylaxis in Children 2 Years and Older

  • 2.5 mg/kg once daily (maximum initial daily dose 200 mg).
  • Dose adjustments should be made according to therapeutic drug monitoring and clinical response.
