✅ Description:
- Phenylephrine: A potent postsynaptic-α-receptor agonist, it causes vasoconstriction in the nasal mucosa, helping to relieve nasal congestion caused by allergies or head colds.
- Promethazine: An H1 receptor blocker, it provides antihistaminic effects along with sedative and antiemetic properties.

Side Effects
- Nervous System: Sedation, sleepiness, blurred vision, dry mouth, dizziness, and rarely, confusion, disorientation, or extrapyramidal symptoms (e.g., oculogyric crisis, torticollis).
- Cardiovascular: Blood pressure changes (increased or decreased), hypertension.
- Dermatologic: Rash, rarely photosensitivity.
- Hematologic: Rare cases of leukopenia, thrombocytopenia, or agranulocytosis.
- Gastrointestinal: Nausea and vomiting.
- Other: Precordial pain, respiratory distress, tremor, and weakness.
Overdose Effects
- Symptoms range from mild CNS and cardiovascular depression to severe hypotension, respiratory depression, unconsciousness, convulsions, cerebral hemorrhage, and vomiting.
- For overdose, monitor vital signs and consider activated charcoal or a cathartic (sodium or magnesium sulfate).
✔️ Dosage & Administration
- Adults and children 12 years and over: 1 teaspoonful (5 mL) every 4 to 6 hours, not to exceed 6 teaspoonsful (30 mL) in 24 hours.
- Children 6 to under 12 years: ½ to 1 teaspoonful (2.5 to 5 mL) every 4 to 6 hours, not to exceed 6 teaspoonsful (30 mL) in 24 hours.
- Children 2 to under 6 years: ¼ to ½ teaspoonful (1.25 to 2.5 mL) every 4 to 6 hours.
- Children under 2 years: Not recommended for use.
- Use as per the guidance of a registered healthcare professional.
✔️ Interaction
- Promethazine: The sedative effects of promethazine can add to the sedative effects of CNS depressants, including alcohol, narcotics, sedatives, and tranquilizers. These drugs should be used with caution or in reduced dosages.
- Phenylephrine:
- MAO inhibitors: Can cause an acute hypertensive crisis.
- Tricyclic antidepressants: May increase pressor response.
- Ergot alkaloids: Excessive rise in blood pressure.
- Bronchodilators and sympathomimetics: May lead to tachycardia or arrhythmias.
- Atropine sulfate: Reflex bradycardia blocked, pressor response enhanced.
- B-blockers (e.g., propranolol): May block the cardio-stimulating effects of phenylephrine.
- Sympathomimetics in diet preparations: Risk of a synergistic adrenergic response.
✔️ Contraindications
- Promethazine: Contraindicated in those with hypersensitivity or a previous reaction to promethazine or other phenothiazines. Not for use in the treatment of lower respiratory tract symptoms, such as asthma.
- Phenylephrine: Contraindicated in patients with hypertension, peripheral vascular insufficiency, or hypersensitivity to the drug. Should not be used in patients taking monoamine oxidase inhibitors (MAOIs).

- Pregnancy Category C: There are no adequate, well-controlled studies in pregnant women. Should only be used if the potential benefit justifies the potential risk to the fetus.
- It is not known if promethazine and phenylephrine are excreted in human milk. Given the potential risks to nursing infants, a decision should be made whether to discontinue nursing or the medication.

- Promethazine: Use with caution in patients with cardiovascular disease, liver impairment, or seizure disorders. Should be avoided in patients with sleep apnea or narrow-angle glaucoma, and those with urinary retention due to prostatic hypertrophy.
- Phenylephrine: Use with caution in patients with thyroid disease, diabetes, cardiovascular disease, or those on tricyclic antidepressants. Caution in elderly or those with poor cerebral or coronary circulation. Risk of urinary retention in men with benign prostatic hypertrophy.
