Folix 15mg Tablet 1 Strip

✔️ Each tablet contains:

• Folinic Acid 5 mg (as Calcium Folinate Hydrate 6.249 mg)
• Folinic Acid 15 mg (as Calcium Folinate Hydrate 18.746 mg)

✔️ Folix is used for:

• Counteracting the toxic effects of folic acid antagonists such as methotrexate
• Calcium folinate rescue therapy during high-dose methotrexate treatment
• Treatment of megaloblastic anemia caused by folate deficiency
• Folate deficiency associated with pregnancy, infancy, liver disease, sprue, and malabsorption syndromes

✔️ Common side effects include:

• Mucositis (inflammation of the mouth lining)
• Stomatitis (mouth sores)
• Leukopenia (low white blood cell count)
• Diarrhea

These effects may become dose-limiting and should be reported to the healthcare provider.

Methotrexate Rescue Therapy

• Treatment should begin as soon as possible, preferably within 24 hours after methotrexate administration.
• A common regimen is 15 mg every 6 hours for 10 doses, although dosing may vary depending on methotrexate levels and clinical response.

Toxicity from Other Folate Antagonists

• For drugs such as trimethoprim or pyrimethamine, 5–15 mg daily may be used to reduce hematologic toxicity.

Methotrexate Overdose

• If methotrexate overdose is suspected, folinic acid should be administered within one hour.
• The folinic acid dose should be equal to or greater than the methotrexate dose.

Megaloblastic Anemia

• 5–15 mg daily according to the severity of deficiency and physician recommendation.

✔️ Interaction

• Large doses of folinic acid may reduce the effectiveness of anticonvulsants such as:
  • Phenytoin
  • Phenobarbital
  • Primidone
• Concurrent use with fluoropyrimidines (such as 5-fluorouracil) may increase the risk of seizures and fainting.
• Folix may reduce the therapeutic effects of methotrexate if given incorrectly.

✔️ Folix should not be used in patients with:

• Hypersensitivity to folinic acid, calcium folinate, or any component of the formulation
• Pernicious anemia or megaloblastic anemia caused by vitamin B12 deficiency
• Rare hereditary conditions such as galactose intolerance, Lapp lactase deficiency, or glucose-galactose malabsorption

✔️ Precautions & Warnings

• Folix should be used with methotrexate or 5-fluorouracil only under specialist supervision.
• In methotrexate overdose, prompt administration is essential; delays beyond 4 hours may reduce effectiveness.
• During methotrexate rescue therapy:
  • Maintain adequate hydration.
  • Alkalinize urine to keep urinary pH above 7.0.
  • Monitor urine output.
  • Regularly assess methotrexate levels, blood urea nitrogen (BUN), and creatinine until methotrexate levels become safe.

✔️ Overdose

No serious toxic effects have been reported with excessive folinic acid doses. However, very high doses may reduce the effectiveness of folic acid antagonist therapies. There is no specific antidote; supportive treatment should be provided if overdose occurs.
