How does Disopan 1mg/ml Injection work?

It works by increasing the action of a chemical messenger (GABA) which suppresses the abnormal and excessive activity of the nerve cells in the brain.

Side Effects of Disopan 1mg/ml Injection

• Common CNS depressant effects include drowsiness, ataxia, behavior changes, and eye movement abnormalities.
• Less frequent: coma, tremor, vertigo, confusion, depression, hallucinations, insomnia, psychosis, palpitations.

• The addiction / habit-forming potential of this medicine is very high. Take it only as per the dose and duration advised by your doctor
• It may cause dizziness. Do not drive or do anything that requires mental focus until you know how this medicine affects you.
• Avoid consuming alcohol as it may increase dizziness and drowsiness.
• Inform your doctor if you experience worsened anxiety, depression anger, or violent behavior and mania while taking this medicine.
• Do not stop taking medication suddenly without talking to your doctor as that may lead to nausea, anxiety, agitation, flu-like symptoms, sweating, tremors, and confusion.

Disopan (Clonazepam) is indicated for the treatment of:

• Panic disorder, with or without agoraphobia, is characterized by unexpected panic attacks and associated concerns.
• Lennox-Gastaut Syndrome (petit mal variant), akinetic and myoclonic seizures.
• Absence of seizures (petit mal) in patients who have not responded to succinimides.

Pharmacology

• Clonazepam, a benzodiazepine, enhances GABAergic neurotransmission, leading to increased GABA receptor affinity and enhanced postsynaptic chloride ion flux.
• Animal and human data indicate clonazepam's rapid suppression of various seizure types, including spike-and-wave discharges.

Dosage of Disopan 1mg/ml Injection

Injection:

• Children: Administer 0.5 mg (half vial) slowly by IV injection or infusion.
• Adults: Administer 1 mg (full vial) slowly by IV; repeat dose as needed, up to 10 mg total, with caution on injection rate (0.25-0.5 mg/min).

Overdose Effects of Disopan 1mg/ml Injection

• Symptoms include severe CNS depression, respiratory depression, and coma.
• Manage with supportive measures; consider activated charcoal and use flumazenil cautiously.

Interaction

No significant alteration of phenytoin, carbamazepine, or phenobarbital pharmacokinetics was noted with Disopan.

Contraindications

Hypersensitivity to benzodiazepines, acute narrow-angle glaucoma, significant liver disease.

Pregnancy & Lactation

• Avoid during pregnancy unless the benefits outweigh the risks; may cause fetal abnormalities.
• Discontinue breastfeeding during treatment due to small amounts excreted in breast milk.

Precautions & Warnings

• Monitor for increased seizure activity in certain seizure disorders.
• Geriatric patients may experience enhanced pharmacologic effects.
• No dosage adjustment is necessary for renal impairment; caution in hepatic impairment.

Storage Conditions:

• Keep Disopan in a dry place away from light and heat.
• Ensure it is stored out of the reach of children.

Before Reconstitution:

• Disopan is typically provided in vials for intravenous administration.
• Store the vials at room temperature (approximately 20°C to 25°C or 68°F to 77°F) unless otherwise specified by the manufacturer.

Reconstitution and Administration:

• For slow intravenous injection, each vial of Disopan (1 mg) should be diluted with 1 mL of water for injection before administration to prevent local irritation of the veins.
• For intravenous infusion, Disopan can be diluted in a ratio of 1 vial (1 mg) to at least 85 mL of diluting media (e.g., sodium chloride 0.9%, glucose 5%). Use infusion bags other than PVC if possible. If PVC bags are used, infuse immediately or within 4 hours.
• Intramuscular injection should only be used in exceptional cases or when intravenous administration is not feasible.

Specific Storage Conditions Post-Reconstitution:

• After reconstitution, follow specific guidelines based on the type of administration:
  • For intravenous infusion, the diluted solution is stable for 24 hours at room temperature.
  • If using PVC bags, infuse immediately or within 4 hours.
  • Do not use sodium bicarbonate solution for preparing Disopan infusions due to potential precipitation.
