Renvista contains roxadustat, an orally administered hypoxia-inducible factor prolyl hydroxylase inhibitor (HIF-PHI). It is indicated for the treatment of symptomatic anemia associated with chronic kidney disease (CKD) in adults.

Renvista is an oral HIF-PH inhibitor that helps stimulate the body's natural production of erythropoietin and supports red blood cell formation. It also improves iron utilization and reduces the effects of inflammation that can interfere with hemoglobin production. By lowering levels of the hormone hepcidin, roxadustat helps increase the availability of iron required for red blood cell production and may improve hemoglobin levels in patients with CKD-associated anemia.

✅ Description:

Roxadustat works by reversibly inhibiting hypoxia-inducible factor prolyl hydroxylase (HIF-PH) enzymes. Under normal oxygen conditions, these enzymes contribute to the breakdown of HIF proteins. Inhibition of HIF-PH allows HIF activity to increase.

Enhanced HIF activity stimulates the production of endogenous erythropoietin (EPO) and other proteins involved in erythropoiesis, iron absorption, iron transport, and hemoglobin synthesis. This promotes the production of red blood cells.

Roxadustat also decreases hepcidin, a hormone that limits iron availability. Lower hepcidin levels can improve the release of stored iron and increase iron absorption and transport, thereby supporting hemoglobin synthesis.

In patients with CKD-related anemia, roxadustat has been shown to increase hemoglobin levels, promote red blood cell production, and improve abnormalities in iron metabolism. It may also reduce cholesterol levels in some patients, irrespective of whether they are receiving statins or other lipid-lowering medicines.

✔️ Dosage & Administration

The dose of roxadustat should be individualized according to the patient's condition and hemoglobin response. It is generally taken three times a week on non-consecutive days, with the treatment goal of maintaining hemoglobin within the target range determined by the treating physician.

Adults not previously receiving erythropoiesis-stimulating agents (ESAs):

  • Usual starting dose: 50 mg three times weekly.
  • Recommended starting dose may be 70 mg three times weekly for patients weighing less than 100 kg.
  • For patients weighing 100 kg or more, the recommended starting dose may be 100 mg three times weekly.

Patients switching from ESA treatment:

  • The usual starting dose is 70 mg or 100 mg three times weekly, depending on the previous ESA treatment and clinical condition.
  • Subsequent doses should be adjusted according to hemoglobin response and the patient's medical status.

Children:
Renvista is not indicated for use in children.

Patients with liver impairment:
Renvista is not recommended in patients with severe hepatic impairment. Patients with liver disease should inform their doctor before starting treatment.

Dose adjustment:

  • Dose changes should be made gradually according to the available dose strengths and the patient's hemoglobin response.
  • Available dose levels include 20 mg, 40 mg, 50 mg, 70 mg, 100 mg, 150 mg, 200 mg, 250 mg and 300 mg; 400 mg is available only for certain dialysis-dependent CKD patients.
  • After a dose adjustment, the new dose should generally be maintained for at least 4 weeks before another adjustment.
  • If hemoglobin rises rapidly by more than 2.0 g/dL within 4 weeks following a dose increase, treatment should be reduced or temporarily interrupted as directed by the physician.

Missed dose:

  • If at least 24 hours remain before the next scheduled dose, take the missed dose as soon as possible and continue the regular schedule.
  • If less than 24 hours remain until the next scheduled dose, skip the missed dose and take the next dose at the usual time.
  • Do not take two doses on the same day.

Method of administration:
Renvista tablets should be taken orally with or without food. Take the medicine exactly according to the prescribed schedule.

✔️ Drug Interactions

Renvista may interact with certain medicines and supplements, potentially altering the amount of roxadustat absorbed or eliminated from the body.

Phosphate binders and products containing multivalent cations:
Some phosphate binders and medicines or supplements containing multivalent cations may significantly reduce roxadustat exposure. Renvista should generally be taken at least 1 hour after these products, except where otherwise directed for lanthanum carbonate.

Gemfibrozil or probenecid:
These medicines may increase roxadustat exposure. Dose adjustment may be necessary based on regular hemoglobin monitoring.

OATP1B1 or BCRP substrates:
Roxadustat may increase exposure to medicines such as simvastatin, rosuvastatin, and atorvastatin. Appropriate monitoring and dose adjustment may be required.

Always inform the doctor about all prescription medicines, over-the-counter products, vitamins, minerals, and supplements being used.

✔️ Pregnancy & Lactation

Roxadustat should not be used during pregnancy and should not be initiated in women who may be pregnant.

Renvista is contraindicated during breastfeeding. Women who are pregnant, planning pregnancy, or breastfeeding should discuss suitable treatment options with their healthcare provider.

✔️ Precautions & Warnings

Renvista should be used carefully and under appropriate medical supervision.

  • It may increase the risk of thrombotic vascular events, particularly in patients who already have risk factors such as obesity or a previous history of thrombosis.
  • Use with caution in patients with a history of seizures.
  • Treatment should be avoided or appropriately managed in patients with serious signs or symptoms of infection.
  • Renvista is not recommended in patients with significant liver disease, particularly severe hepatic impairment.
  • It should not be initiated during pregnancy.
  • Hemoglobin levels should be monitored regularly to guide dose adjustment and minimize the risk associated with excessive or rapid increases in hemoglobin.
