Histamine acts on H1 receptors and plays a key role in allergic responses. When allergens trigger mast cells and basophils, histamine is released, leading to symptoms such as itching, sneezing, watery eyes, and nasal discharge.
Fexofenadine is a selective H1-receptor antagonist (inverse agonist). It stabilizes the inactive form of the H1 receptor, preventing histamine from activating allergic responses. Unlike many antihistamines, it does not cause sedation because it does not cross the blood–brain barrier and has no significant anticholinergic or CNS effects.
