Linagliptin is a DPP-4 inhibitor that enhances glycemic control in type 2 diabetes. It works by blocking the enzyme dipeptidyl peptidase-4, which normally breaks down incretin hormones such as GLP-1 and GIP. By increasing active incretin levels, it promotes glucose-dependent insulin secretion from pancreatic beta cells and reduces glucagon release from alpha cells.
Metformin hydrochloride is a biguanide oral antidiabetic agent that lowers blood glucose without causing hypoglycemia. It acts by reducing hepatic glucose production, decreasing intestinal glucose absorption, and improving insulin sensitivity through increased peripheral glucose uptake and utilization.
The dose should be individualized based on efficacy and tolerability. The maximum recommended daily dose is linagliptin 5 mg and metformin 2000 mg, preferably taken with meals.

Symptoms of lactic acidosis include abdominal discomfort, muscle pain, rapid breathing, weakness, and unusual fatigue. Immediate medical attention is required if these occur.
