Linagliptin acts as a DPP-4 (dipeptidyl peptidase-4) inhibitor, an enzyme responsible for breaking down incretin hormones such as GLP-1 and GIP. - Maximum dose: 2.5 mg Linagliptin + 1000 mg Metformin twice daily with meals.
- Initial dose for new patients: 2.5 mg Linagliptin + 500 mg Metformin twice daily.
- Patients already on Metformin: Start with 2.5 mg Linagliptin + current Metformin dose twice daily.
- Patients switching from separate Linagliptin and Metformin: May transition to an equivalent fixed-dose combination.

- Maximum daily dose: Linagliptin 5 mg + Metformin 2000 mg once daily with a meal.
- Starting dose (new patients): 5 mg Linagliptin + 1000 mg Metformin ER once daily with food.
- Patients already on Metformin: Use 5 mg Linagliptin + equivalent Metformin total daily dose once daily.

- Supportive care including GI decontamination and monitoring is recommended.
- Linagliptin is not effectively removed by dialysis.
