Lotepred contains loteprednol, a corticosteroid used to manage steroid-responsive inflammation of the eye. It helps reduce redness, swelling, itching, irritation, and discomfort by suppressing the release of inflammatory substances in the eye.
Do not use Lotepred if the bottle seal is damaged or broken before first use. Wash your hands before applying the drops and avoid touching the dropper tip to your eye or any surface. Contact lens users should remove their lenses before application and wait at least 15 minutes before reinserting them.
Do not stop treatment earlier than recommended by your doctor, as premature discontinuation may allow inflammation or infection to return. Temporary burning, irritation, watery eyes, or blurred vision may occur after application. Avoid driving or operating machinery until your vision becomes clear.
Most local eye reactions are temporary. Contact your doctor if symptoms persist or become troublesome.
Loteprednol is a synthetic, non-fluorinated glucocorticoid. It reduces inflammation by influencing proteins involved in the production and activity of inflammatory mediators such as prostaglandins and leukotrienes. This helps decrease redness, swelling, itching, and other inflammatory symptoms.
Apply the missed dose as soon as you remember. If it is nearly time for your next scheduled dose, skip the missed application and continue with your usual dosing schedule. Do not apply extra drops to make up for the missed dose.
For steroid-responsive inflammatory eye conditions:
Instill 1–2 drops into the conjunctival sac of the affected eye(s) four times daily (QID). In more severe cases, the dose may be increased to 1 drop every hour during the initial treatment period, particularly during the first week, as directed by the ophthalmologist. Patients should be reassessed if symptoms do not improve within 2 days.
For postoperative ocular inflammation:
Instill 1–2 drops into the operated eye(s) four times daily, beginning 24 hours after surgery and continuing for up to 2 weeks, according to the doctor's instructions.
Eye ointment:
A small amount, approximately a 0.5-inch ribbon, may be placed inside the conjunctival sac four times daily, starting 24 hours after surgery and continuing for up to 2 weeks when prescribed.
The safety and effectiveness of loteprednol in children have not been adequately established. Use in children only under appropriate medical supervision.

Lotepred can suppress symptoms and potentially worsen an existing eye infection. Patients should be reassessed if symptoms fail to improve within 2 days.
Prolonged corticosteroid use may increase intraocular pressure and contribute to glaucoma or optic nerve damage. It may also delay healing following cataract surgery and, with extended use, increase the risk of posterior subcapsular cataract formation.
Particular caution is required in patients with glaucoma or conditions associated with corneal or scleral thinning because prolonged steroid treatment may rarely contribute to perforation of the eye.
Possible adverse effects include:
Long-term use may cause increased intraocular pressure, glaucoma, optic nerve damage, posterior subcapsular cataract, delayed wound healing, and, rarely, perforation in eyes with marked corneal or scleral thinning.
If you are pregnant, planning to become pregnant, or breastfeeding, consult your doctor before using Lotepred. The medicine should be used during pregnancy or lactation only when considered necessary by a healthcare professional.
Store the medicine according to the manufacturer's recommended conditions, generally away from excessive heat, moisture, and direct light. Keep out of reach of children. Once opened, use within the recommended period, commonly 4 weeks.
