✅ Description:

Chemotherapy-Induced Nausea and Vomiting (CINV): CINV is a common and severe side effect of chemotherapy. Ondansetron helps alleviate nausea and vomiting associated with cancer treatment by blocking serotonin receptors that trigger these symptoms.

Postoperative Nausea and Vomiting (PONV): PONV occurs shortly after surgery, often due to anesthesia. Ondansetron is effective in treating and preventing nausea and vomiting in the postoperative period.

Radiation-Induced Nausea and Vomiting (RINV): RINV affects cancer patients undergoing radiotherapy. Ondansetron helps manage nausea and vomiting during and shortly after radiotherapy sessions.

✔️ Uses of Seton 8mg Tablet
  • Used to manage nausea and vomiting induced by chemotherapy, radiotherapy, and surgery
✔️ How does Seton 8mg Tablet work?

It works by blocking the action of a chemical messenger (serotonin) in the brain that may cause nausea and vomiting during anti-cancer treatment (chemotherapy) or after surgery.

✔️ Side Effects of Seton 8mg Tablet

The most common side effects of Ondansetron are constipation, diarrhea, fatigue, and headache. However, these are usually not bothersome and resolve on their own after some time. Consult your doctor if these persist or worry you.

✔️ Directions for Use:

Take Seton 8mg Tablet as advised by your physician. It can be taken with or without food. Swallow Seton 8mg Tablet with a glass of water. Do not crush or chew the medicine.

Your doctor will decide the correct dose and duration of therapy for you depending on your age, body weight, and severity of the infection.

✔️ Overdosage:

If you or anyone else accidentally takes too much of Seton 8mg Tablet, consult your doctor immediately or visit the nearby hospital.

✔️ Missed Dose

Ondansetron is taken as a single dose, hence the chances of missing a dose are very low. If you are on a scheduled dosing regimen of this medicine, take the missed dose as soon as you remember. If it is almost time for the next dose, skip the missed dose. Do not double the dose to make up for the missed one.

✔️ Adult Dosages:

Prevention of nausea & vomiting associated with chemotherapy:

  • Highly emetogenic chemotherapy: 24 mg once, 30 minutes before chemotherapy.
  • Moderately emetogenic chemotherapy: 8 mg before chemotherapy, then 8 mg every 8 hours for 1-2 days.

Prevention of nausea & vomiting associated with radiotherapy:

  • Total body irradiation: 8 mg 1-2 hours before each session.
  • Single high-dose fraction radiotherapy to the abdomen: 8 mg before, then 8 mg every 8 hours for 1-2 days.
  • Daily fractionated radiotherapy to the abdomen: 8 mg before, then 8 mg every 8 hours for each day.

Prevention of post-operative nausea & vomiting: 16 mg 1 hour before anesthesia induction.

Nausea-vomiting in gastroenteritis: 8 mg three times daily.

Nausea-vomiting in pregnancy: 8 mg 2-3 times daily.

✔️ Quick Suggestions:
  • You have been prescribed Ondansetron for the prevention of nausea and vomiting caused after surgery or due to chemotherapy and radiotherapy.
  • It is fast-acting and starts working within 30 minutes.
  • If you vomit within one hour of taking a dose, take another dose.
  • Avoid heavy meals and try eating small nourishing snacks throughout the day. Also, sip water regularly to help avoid dehydration.
✔️ Pediatric Dosages:

Moderate emetogenic cancer chemotherapy (Child 4-11 years):

  • 4 mg tablet before chemotherapy, then 4 mg at 4 and 8 hours after the first dose. Then 4 mg three times a day for 1-2 days post-chemotherapy.

Prevention of nausea & vomiting associated with radiotherapy (Child >12 years):

  • Similar to adult dosages.

Nausea-vomiting in gastroenteritis (Children >1 month): 0.15 mg/kg three times daily.

Prevention of post-operative nausea & vomiting (Child >12 years): 16 mg 1 hour before anesthesia.

✔️ Special Populations:

Elderly: No specific dosage adjustment was mentioned.

Severe hepatic impairment (Child-Pugh score ≥10): Maximum 8 mg/day.

Renal impairment: No dose adjustment necessary.

✔️ Pharmacology

Chemotherapy-Induced Nausea and Vomiting (CINV) and Radiotherapy-Induced Nausea and Vomiting (RINV):

  • Chemotherapeutic agents and radiotherapy can induce the release of serotonin (5HT) in the small intestine. This release activates 5HT3 receptors on vagal afferents, triggering a vomiting reflex.
  • Ondansetron blocks these 5HT3 receptors, thereby inhibiting the initiation of the vomiting reflex in the peripheral nervous system.
  • Additionally, serotonin release in the area postrema (located in the brainstem) can also contribute to emesis through central mechanisms. Ondansetron's action on 5HT3 receptors in the central nervous system helps mitigate this central pathway of nausea and vomiting.

Post-Operative Nausea and Vomiting (PONV):

  • The exact mechanisms of PONV are not fully elucidated, but they may involve pathways similar to those in CINV and RINV.
  • Ondansetron's ability to block 5HT3 receptors in both peripheral and central nervous systems suggests it can intervene in the common pathways that lead to postoperative nausea and vomiting.
✔️ Interaction
  • Apomorphine: Both ondansetron and apomorphine can affect serotonin levels in the brain. Concurrent use can lead to serotonin syndrome, characterized by symptoms such as agitation, confusion, rapid heart rate, and high blood pressure.
  • Antiepileptic drugs (e.g., phenytoin, carbamazepine): Ondansetron may reduce the effectiveness of these medications due to its potential to induce hepatic enzymes that metabolize them. This could lead to decreased seizure control.
  • Antituberculosis drugs (e.g., rifampicin): Rifampicin induces hepatic enzymes that can accelerate the metabolism of ondansetron, potentially reducing its efficacy.
  • Opioids (e.g., tramadol, methadone): Ondansetron may enhance the analgesic effects of opioids and can also increase the risk of serotonin syndrome when used together.
  • Antipsychotics (e.g., haloperidol): Concurrent use with ondansetron can increase the risk of QT prolongation and potentially life-threatening arrhythmias.
  • Cardiac medications (e.g., amiodarone, atenolol, timolol): Ondansetron may also prolong the QT interval, and when used with other QT-prolonging medications, it can increase the risk of arrhythmias.
  • Cancer medications (e.g., doxorubicin, trastuzumab): Ondansetron may alter the metabolism of these drugs or interfere with their therapeutic effects.
  • Antimicrobials (e.g., erythromycin, ketoconazole): These drugs can inhibit the metabolism of ondansetron, leading to increased blood levels and potentially higher risk of side effects.
  • Antidepressants and anxiolytics (e.g., fluoxetine, paroxetine, sertraline, duloxetine): Combining these drugs with ondansetron can increase serotonin levels, potentially leading to serotonin syndrome.
