Chemotherapy-induced N/V: 8–24 mg/day (tablet/film/solution/injection, per protocol)

Radiotherapy-induced N/V: 8 mg orally, 1–2 hours before therapy, then every 8 hours as needed

Postoperative N/V: 16 mg orally (tablet/film) or 4 mg injection before anesthesia

Pediatrics: Weight- and age-adjusted dosing; oral films available in 4 mg and 8 mg strengths
