✔️ Dosage:

Oral Dosage

• Adults: 10-20 mg (1-2 tablets or 10-20 ml suspension) every 6-8 hours (Max: 80 mg/day).
• Children: 2-4 ml suspension/10 kg or 0.4-0.8 ml pediatric drops/10 kg every 6-8 hours.

For Dyspepsia:

• Adults: 10-20 mg every 6-8 hours.
• Children: 0.2-0.4 mg/kg every 6-8 hours.

For Acute & Sub-Acute Nausea/Vomiting:

• Adults: 20 mg every 6-8 hours.
• Children: 0.2-0.4 mg/kg every 6-8 hours (Max treatment period: 12 weeks).

Rectal Administration (Suppositories):

• Adults: 30-60 mg every 4-8 hours.
• Children (10-25 kg): Max 30 mg/day, divided into smaller doses if required.

⚠ Do not use for more than 12 weeks without medical supervision.

✔️ Administration

Administer Domperidone 15-30 minutes before meals. If necessary, a dose may be taken before bedtime.

✔️ Side Effects

• Occasional transient intestinal cramps
• Neurological: Rare extrapyramidal effects, more common in young children
• Endocrine: Increased plasma prolactin levels, potentially causing:
• Galactorrhea
• Gynecomastia
• Allergic reactions: Rash, urticaria

✔️ Drug Interactions

• Anticholinergic drugs may counteract the antidyspeptic effect.
• Antacids & antisecretory drugs lower Domperidone’s oral bioavailability.
• Metabolized via CYP3A4 enzyme: Inhibitors such as azole antifungals, macrolide antibiotics, HIV protease inhibitors, and nefazodone may increase Domperidone levels.
• Possible altered absorption of enteric-coated or sustained-release medications.
• Compatible with: Neuroleptics and dopaminergic agonists (bromocriptine, L-dopa), reducing their gastrointestinal side effects without affecting central actions.

✔️ Contraindications

• Hypersensitivity to Domperidone
• Neonates
• Conditions where gastrointestinal stimulation is dangerous, such as:
• Gastrointestinal bleeding
• Mechanical obstruction
• Perforation
• Prolactin-releasing pituitary tumors (prolactinoma)

✔️ Overdose Management

Symptoms: 

• Drowsiness, disorientation, extrapyramidal reactions (especially in children).

Treatment:

• Activated charcoal
• Symptomatic monitoring
• Anticholinergic drugs (e.g., antiparkinson agents, antihistamines) for extrapyramidal effects

✔️ Pregnancy & Lactation

• Pregnancy: Animal studies show no teratogenic effects, but use in the first trimester is advised only if the benefits outweigh the risks.
• Lactation: Excreted in breast milk in low concentrations. Nursing is not recommended unless necessary.

✔️ Precautions & Warnings

Children: 

• Use cautiously due to the risk of extrapyramidal side effects.

Hepatic Impairment: 

• As Domperidone is metabolized in the liver, it should be used with caution in patients with liver disease.

Renal Impairment:

• Severe renal insufficiency increases drug half-life from 7.4 to 20.8 hours.

Adjust dosage frequency (once or twice daily instead of multiple doses).
