✔️ Drug Interactions

• May reduce the effects of levodopa and dopamine agonists
• Drugs inducing CYP1A2 or glucuronyl transferase (e.g., omeprazole, rifampicin) may increase clearance of Xytrex
• CYP1A2 inhibitors may reduce Xytrex elimination
• Carbamazepine may increase clearance
• Activated charcoal reduces oral bioavailability by 50–60%
• Use caution with centrally acting drugs and alcohol

✔️ Side Effects

Very common:

• Somnolence (sleepiness)
• Weight gain

Other possible effects:

• Increased appetite
• Elevated glucose and triglycerides
• Dizziness
• Akathisia (restlessness)
• Parkinsonian symptoms or dyskinesia
• Orthostatic hypotension
• Mild anticholinergic effects: constipation, dry mouth
• Asthenia (weakness), edema, photosensitivity

✔️ Pharmacology

Olanzapine is an atypical antipsychotic that interacts with:

Serotonin receptors: 5HT2A/2C, 5HT3, 5HT6

Dopamine receptors: D1–D5

Cholinergic muscarinic receptors: M1–M5

α1 adrenergic and histamine H1 receptors

Mechanism of Action:
The precise mechanism is not fully understood. It is believed that Xytrex’s effects in schizophrenia are due to a combination of dopamine and serotonin 5-HT2 receptor antagonism.

Pharmacokinetics:

Well absorbed orally, peak plasma levels in 5–8 hours

Food does not affect absorption

Not mutagenic, clastogenic, or carcinogenic

1. Children (<18 years): Not studied
2. Elderly (>65 years): Start at 5 mg/day
3. Hepatic or renal impairment: Start at 5 mg/day

✔️ Administration

Can be taken with or without food

Dose tapering is recommended when discontinuing treatment

✔️ Dosage

Schizophrenia or bipolar disorder (combination therapy):

• Initial: 5–10 mg once daily
• Adjust according to clinical response, range: 5–20 mg/day
• Doses above 10 mg/day should follow reassessment
• Maximum: 20 mg/day

Monotherapy for mania:

• Initial: 15 mg once daily
• Adjust according to response, range: 5–20 mg/day
• Doses above 15 mg/day require reassessment
• Maximum: 20 mg/day

Use only under physician supervision.

✔️ Contraindications:

Hypersensitivity to olanzapine or any component of the product

Known risk for narrow-angle glaucoma

✔️ Pregnancy & Lactation

• Use during pregnancy only if the benefits outweigh the risks
• Notify your physician if pregnant or planning a pregnancy
• Breastfeeding is not recommended while taking Xytrex

✔️ Use cautiously in patients with:

• Seizure history or risk of seizures
• Low leukocyte/neutrophil counts or drug-induced bone marrow suppression
• Hypereosinophilic conditions
• Liver impairment or hepatotoxic drug use
• Drugs that prolong QT interval
• Elderly patients require caution
• Avoid operating machinery or driving if dizziness occurs
