✅ Description:
Apremilast is an oral phosphodiesterase-4 (PDE4) inhibitor.
By blocking PDE4, it raises intracellular cAMP levels, leading to a reduction in proinflammatory cytokines such as:
- TNF-α
- IL-2
- Interferon-γ
- Leukotrienes
- Nitric oxide synthase
This helps rebalance the immune system’s inflammatory response by modulating multiple cytokine pathways, rather than targeting a single one.

Initial Dose Titration (Days 1–5):

Depression & Mood Changes: Use with caution in patients with a history of depression or suicidal thoughts. Monitor for mood shifts.
Weight Loss: Unintentional weight loss (5–10%) may occur. Monitor body weight during treatment.
Caregivers should report any changes in mood, appetite, or mental health promptly.
