✔️ Side Effects

Common:

• Dry mouth
• Blurred vision
• Dizziness
• Mild nausea
• Nervousness

Less Common but Serious:

• Confusion, hallucinations, or paranoia
• Constipation, urinary retention
• Increased heart rate
• Eye pressure increase (glaucoma risk)
• Skin rash or dryness

Abrupt withdrawal may worsen Parkinsonism or cause neuroleptic malignant syndrome (NMS).

✔️ Interaction

• Alcohol, barbiturates, opioids, and cannabinoids may enhance sedative effects
• MAO inhibitors and tricyclic antidepressants may increase anticholinergic side effects
• Concomitant use with neuroleptics may increase risk of tardive dyskinesia
• Dose adjustments may be required when used with levodopa

✔️ Dosage & Administration

• Dose should be individualized and started at a low level, then gradually increased—especially in elderly patients (>60 years).
• It may be taken before or after meals, depending on patient tolerance.
• If excessive dry mouth occurs, taking it before meals may help; if nausea occurs, taking it after meals is preferable.

Parkinsonism (Idiopathic):

• Initial dose: 1 mg on the first day
• Increase by 2 mg every 3–5 days
• Usual total daily dose: 6–10 mg (some may require up to 12–15 mg)

Drug-induced Parkinsonism:

• Dose varies (typically 5–15 mg/day)
• Start with 1 mg and increase gradually until symptoms are controlled

With Levodopa:

• Dose of both drugs may need adjustment

General Advice:

• Divide the daily dose into 3 portions with meals
• High doses (>10 mg/day) may be divided into 4 doses (last dose at bedtime)
• Avoid abrupt discontinuation (may worsen symptoms or cause NMS)

✔️ Contraindications

• Hypersensitivity to trihexyphenidyl
• Narrow-angle glaucoma (risk of vision loss)

✔️ Overdose Effects

Symptoms:

• Dry, flushed skin
• Dilated pupils
• Fever, tachycardia
• Confusion, hallucinations, seizures

Management:

• Supportive care
• Gastric lavage if needed
• Sedatives (e.g., diazepam) for CNS excitation
• Maintain hydration and temperature control

✔️ Pregnancy & Lactation

• No adequate studies in humans or animals
• Use only if the benefits outweigh the risks
• May suppress lactation; caution in breastfeeding mothers

✔️ Precautions & Warnings

• Monitor intraocular pressure (risk of glaucoma)
• Use cautiously in hot environments (risk of heatstroke due to reduced sweating)
• Increased risk of hyperthermia in patients with impaired sweating
• Monitor for signs of NMS (fever, rigidity, altered mental status)

• Keep in a dry place, protected from light
