Pharmacology

• Tofacitinib is a partially reversible JAK inhibitor that prevents the body from responding to cytokine signals.
• It inhibits JAK, preventing STAT phosphorylation and activation, and works as an anti-inflammatory agent by inhibiting the JAK-STAT pathway.
• The JAK-STAT signaling pathway regulates cell transcription in hematopoietic and immune cell functions.

Dosage & Administration of Tofanib XR

• For rheumatoid arthritis: Tofacitinib 5 mg twice daily or Tofacitinib 11 mg once daily.
• For psoriatic arthritis: Tofacitinib 5 mg twice daily or Tofacitinib 11 mg once daily (in combination with nonbiologic DMARDs).
• For ulcerative colitis: Tofacitinib 10 mg twice daily for at least 8 weeks, followed by 5 or 10 mg twice daily.

Interaction

• Interaction with CYP3A4 inhibitors (e.g., ketoconazole) may require dose adjustment.
• Cautious use of medications causes CYP3A4 inhibition and CYP2C19 inhibition.
• Rifampin and other CYP inducers may reduce clinical response.

Contraindications

Hypersensitivity to the active substance or any of the excipients.

Pregnancy & Lactation

• Pregnancy category C. Use during pregnancy only if potential benefits justify potential risks.
• Unknown if tofacitinib is excreted in human milk; the decision to stop breastfeeding or the drug should consider the importance of the drug for breastfeeding.

Precautions & Warnings

• Avoid use during active severe infections.
• Use with caution in patients at high risk of gastrointestinal perforation.
• Regular laboratory testing is advised due to potential changes in lymphocytes, neutrophils, hemoglobin, liver enzymes, and lipids.
• Avoid use with live vaccines.
• Consideration of absolute lymphocyte count (ALC), absolute neutrophil count (ANC), and hemoglobin levels is necessary.
