Tolfenamic acid is a fenamate NSAID that inhibits cyclooxygenase (COX) enzymes, thereby reducing the synthesis of inflammatory mediators such as prostaglandin E2 (PGE2) and thromboxane B2 (TXB2). This action decreases pain, swelling, and inflammation. Additionally, it has a direct antagonistic effect on prostaglandin receptors.

Absorption: Rapid from the gastrointestinal tract; peak plasma concentration in 60–90 minutes
Bioavailability: ~85%
Distribution: 99% protein-bound
Half-life: 2 hours
Metabolism: Liver; undergoes enterohepatic circulation
Excretion: Mainly via urine (~90%) and feces
Adults:
Acute migraine attacks: 200 mg at onset, may repeat once after 1–2 hours
Mild to moderate pain: 100–200 mg three times daily (tid)
Renal Impairment: Dose adjustment may be needed; avoid in severe renal impairment
Children: Pediatric dosing not established
Administration: Take with food and sufficient water
Active gastrointestinal bleeding or ulceration
History of NSAID-related gastrointestinal hemorrhage or perforation
Severe heart failure
Overdose symptoms: Headache, nausea, vomiting, GI bleeding, drowsiness, dizziness, convulsions; severe cases may cause acute renal failure or liver damage
Absorption: Increased with Metoclopramide and Magnesium hydroxide; decreased with Aluminium hydroxide
Caution with: Anticoagulants, other NSAIDs (increased bleeding risk), lithium, methotrexate, cardiac glycosides, ACE inhibitors, diuretics, ciclosporin, tacrolimus (risk of nephrotoxicity)
May reduce the efficacy of loop diuretics, beta-blockers, and ACE inhibitors
Avoid during pregnancy unless essential; do not use in the third trimester
NSAIDs may appear in breast milk; avoid if possible during breastfeeding
Use cautiously in patients with: asthma, bronchospasm, bleeding disorders, cardiovascular disease, hypertension, liver or renal impairment, peptic ulcer, and the elderly
Increase water intake or adjust dose to reduce risk of dysuria
Seek immediate medical attention if severe reactions or overdose occur
Store in a cool, dry place away from light
Keep out of the reach of children
