Finerenone is indicated to reduce the risk of kidney and cardiovascular complications in adults with chronic kidney disease (CKD) associated with type 2 diabetes. It helps lower the risk of sustained decline in estimated glomerular filtration rate (eGFR), end-stage kidney disease, cardiovascular death, nonfatal myocardial infarction, and hospitalization for heart failure.

Finerenone is a selective, nonsteroidal mineralocorticoid receptor antagonist (MRA). It blocks the effects of aldosterone and cortisol at mineralocorticoid receptors, which regulate gene activity in various tissues, including the kidneys, heart, and blood vessels.
Overactivation of these receptors may contribute to inflammation, tissue damage, and fibrosis. By blocking mineralocorticoid receptor activity, finerenone helps protect kidney and cardiovascular health. It has a high affinity for mineralocorticoid receptors and does not significantly interact with androgen, progesterone, estrogen, or glucocorticoid receptors.
Treatment with finerenone may produce a modest reduction in blood pressure, typically around 3 mmHg in systolic pressure and 1–2 mmHg in diastolic pressure.
Absorption: Finerenone is rapidly absorbed after oral administration, reaching peak plasma concentrations approximately 30 minutes to 1.25 hours after dosing. Its absolute bioavailability is about 44%. Food does not have a clinically significant effect on overall drug exposure.
Distribution: The steady-state volume of distribution is approximately 52.6 litres. About 92% of finerenone binds to plasma proteins, primarily albumin.
Metabolism: Finerenone is mainly metabolized by the CYP3A4 enzyme, accounting for approximately 90% of its metabolism, with CYP2C8 contributing the remaining 10%. The resulting metabolites are inactive.
Elimination: The terminal half-life is approximately 2–3 hours, with systemic clearance of around 25 litres per hour. Approximately 80% of the administered dose is excreted through the urine.
Steady state: Finerenone generally reaches steady-state concentrations after approximately two days of daily administration.

Recommended starting dose based on eGFR:
The dose may be increased after four weeks, depending on serum potassium and kidney function, to reach 20 mg once daily when appropriate.
Finerenone can be taken with or without food. If swallowing a tablet is difficult, it may be crushed and mixed with water or soft food, according to the product instructions.
Adverse reactions reported more frequently with finerenone than with placebo include:
Measure serum potassium and eGFR before starting treatment. Do not initiate finerenone if serum potassium is above 5.0 mEq/L.
Check serum potassium approximately four weeks after starting treatment and again four weeks after any dose adjustment. Continue periodic monitoring throughout treatment, with more frequent testing when clinically necessary.
If potassium levels rise, the dose may need to be reduced or treatment temporarily withheld according to the prescribing guidelines.
Pregnancy: There are limited human data regarding finerenone use during pregnancy. Animal studies have reported developmental effects at exposures higher than those expected with standard human dosing. Use during pregnancy should be discussed with a healthcare professional after carefully assessing potential risks and benefits.
Breastfeeding: It is not known whether finerenone or its metabolites pass into human breast milk. Because of the potential risk to a breastfed infant, breastfeeding should be avoided during treatment and for one day after the last dose, according to the prescribing information.
