• Elderly: No dose adjustment required with normal renal function

✔️ Pharmacokinetics

Absorption:

• Rapid and well absorbed after oral administration
• Absolute bioavailability ~92%
• Peak plasma concentration achieved within 3–6 hours
• Food or milk does not affect absorption
• Antacids containing aluminium or magnesium may reduce absorption by ~50%

Distribution:

• Widely distributed in body tissues (Vd ≈ 3.9 L/kg)
• Plasma protein binding ~45%
• High penetration into respiratory tissues (3–6 times higher than plasma levels)
• Accumulates in alveolar macrophages

Metabolism:

• Primarily metabolized in the liver via glucuronidation (Phase II)
• Does not significantly involve cytochrome P450 enzymes

Excretion:

• Eliminated via urine (50%) and feces (50%)
• About 10% excreted unchanged in urine
• Total clearance: ~11.4 L/hr

✔️ Dosage & Administration

Patients with normal renal function:

• Loading dose: 400 mg (two 200 mg tablets) on Day 1
• Maintenance dose: 200 mg once daily for 10 days

Patients with renal impairment (CrCl <50 mL/min):

• Loading dose: 400 mg on Day 1
• Maintenance dose: 200 mg every 48 hours for 9 days

✔️ Drug Interactions

• Antacids containing aluminium or magnesium and sucralfate reduce absorption due to chelation
• Avoid concomitant use with drugs that prolong QT interval (e.g., terfenadine)
• No significant interaction with theophylline, caffeine, warfarin, cimetidine, or probenecid

✔️ Contraindications

• Hypersensitivity to sparfloxacin or other quinolones
• Pregnancy and lactation
• Glucose-6-Phosphate Dehydrogenase Deficiency
• History of fluoroquinolone-associated tendon disorders (e.g., Achilles tendinitis)

✔️ Warnings:

Special Warnings:

• Risk of phototoxicity—avoid sunlight and UV exposure
• Discontinue immediately if skin reactions occur (burning, rash, swelling, itching)
• May prolong QT interval—use cautiously in at-risk patients
