✔️ Words of Advice:

• Take at the same time daily, with food.
• Avoid alcohol during therapy.
• Stand up slowly to prevent dizziness or fainting.
• Do not stop abruptly—can worsen the condition.
• Inform the doctor if you are pregnant, breastfeeding, or have heart/liver issues.
• May impair alertness—avoid driving until you know how it affects you.

✔️ Common Side Effects

• Headache
• Dizziness
• Constipation
• Peripheral edema (swelling in ankles/feet)
• Nausea
• Fatigue
• Rash
• Flushing
• Sleep disturbances
• Less common:
• Gingival hyperplasia
• Increased liver enzymes
• Hypotension
• Dyspnea

✔️ Dosage & Administration

Adults

Hypertension (Immediate-release):

• Start: 80 mg orally every 8 hours
• Maintenance: 80–320 mg/day divided q8h

Hypertension (Extended-release):

• Start: 180 mg once daily (lower if elderly/small frame)
• Max: 480 mg/day

Angina:

• IR: 80–120 mg every 8 hours, up to 480 mg/day
• ER: 180–480 mg/day at bedtime

Arrhythmias:

• 240–480 mg/day in divided doses

✔️ Drug Interactions

• Increased bradycardia & conduction issues: Beta-blockers, digoxin, clonidine
• Increased verapamil levels: Protease inhibitors, cimetidine
• Decreased verapamil levels: Rifampin, phenobarbital
• Increased levels of other drugs: Doxorubicin, cyclosporine, statins, buspirone
• Alcohol: May increase its blood levels
• Potentially fatal: Combined use with amiodarone, dofetilide, dantrolene, etc.

✔️ Contraindications

• Severe hypotension (SBP <90 mmHg)
• Cardiogenic shock
• Marked bradycardia
• 2nd or 3rd degree AV block (without pacemaker)
• Sick sinus syndrome
• Uncompensated heart failure
• WPW or LGL syndromes with atrial arrhythmias

✔️ Precautions & Warnings

• Caution in bradycardia, first-degree AV block, hepatic or renal impairment
• Avoid abrupt withdrawal
• Monitor: BP, heart rate, ECG, Liver function tests
• Use in pregnancy and lactation only if clearly needed (distributed in breast milk)
