• Common side effects (1-10% incidence) include headache, fatigue, dizziness, diarrhea, nausea, increased triglyceride levels, insulin resistance, decreased high-density lipoprotein (HDL) levels, insomnia, peripheral edema, and weakness.
• Less common side effects (<1% incidence) may include bradycardia, chest pain, dyspnea, and potentially fatal anaphylaxis.

Indications:

Nebita 2.5mg Tablet is used in the treatment of essential hypertension (high blood pressure). It also controls heart pumping strength and in the treatment of mild and moderate chronic heart failure (above 70 years of age).

Pharmacology

Pharmacodynamics:

• Nebivolol has very high selectivity for β-1 adrenergic receptors. This means it primarily targets and blocks these receptors in the body.
• It also has a nitric oxide-mediated vasodilatory effect. This means it can relax and widen blood vessels by promoting the release of nitric oxide. This vasodilatory action helps reduce blood pressure and improve blood flow.

Pharmacokinetics:

• Nebivolol is metabolized in the body through various routes, including glucuronidation (a process in which the drug is conjugated with glucuronic acid) and hydroxylation by the CYP2D6 enzyme system.
• The active isomer of Nebivolol, d-Nebivolol, has an effective half-life of about 12 hours in extensive metabolizers, which includes most people. This means that it takes about 12 hours for half of the drug to be eliminated from the body.
• In poor metabolizers, the effective half-life of d-Nebivolol is longer, approximately 19 hours.
• Peak plasma concentrations of Nebivolol occur approximately 1.5 to 4 hours after administration.
• Nebivolol binds strongly to plasma proteins, with approximately 98% of the drug binding to albumin. This binding affects how the drug is distributed and eliminated in the body.

Dosage & Administration of Nebita 2.5mg Tablet

For hypertension:

• Adults: Initially, 5 mg once daily. The dose may be increased at intervals of 2 weeks if needed, with a maximum dose of 40 mg once daily.
• Elderly (>65 years): Initially, 2.5 mg once daily, increased to 5 mg once daily if required.

For heart failure:

• Adults: Initially, 1.25 mg once daily. The dose may be doubled every 1-2 weeks if tolerated, with a maximum dose of 10 mg once daily.

How to Use Nebita 5:

• Take this medicine exactly as prescribed by your doctor.
• Swallow the tablet whole; do not chew, crush, or break it.
• You can take Nebita 5 with or without food, but it's advisable to take it at the same time every day.

Interaction

Nebita 5 can interact with other medications, such as potent CYP2D6 inhibitors, antiarrhythmic drugs, calcium channel blockers, beta-adrenergic blocking agents, digoxin, catecholamine-depleting agents, and clonidine.

Contraindications

Nebita 5 is contraindicated in individuals with certain conditions, including severe hepatic impairment, sick sinus syndrome, certain heart blockages, asthma, metabolic acidosis, severe peripheral arterial disease, severe bradycardia, cardiogenic shock or decompensated heart failure, untreated phaeochromocytoma, and during pregnancy and lactation.

Pregnancy & Lactation

The use of β-blockers like Nebita 5 during pregnancy may lead to intrauterine growth restriction, neonatal hypoglycemia, and bradycardia, especially in cases of severe hypertension. Close monitoring of infants is advised if β-blockers are used close to delivery. Nebita 5 is not recommended during breastfeeding due to potential toxicity risks.

Precautions & Warnings

• Nebita 5 should be used with caution in patients with inadequate cardiac function, well-compensated heart failure, and myasthenia gravis.
• It may mask signs and symptoms of hypoglycemia and hyperthyroidism.
• Abrupt withdrawal may exacerbate angina symptoms and increase the risk of myocardial infarction (heart attack) and ventricular arrhythmias in patients with coronary artery disease.
• Nebita 5 is not recommended during pregnancy and lactation.
