✔️ Pharmacology

• Clonidine Hydrochloride stimulates alpha-adrenoceptors in the brainstem, reducing blood pressure within 30-60 minutes after oral administration.
• Maximum BP reduction occurs within 2-4 hours.
• Renal function remains stable; postural reflexes are intact, making orthostatic hypotension rare.

✔️ Overdose Effects

⚠ Symptoms:

• Severe hypotension, bradycardia, respiratory depression, hypothermia, sedation, weakness, abnormal reflexes, pinpoint pupils (miosis).

✔️ Dosage & Administration

Adults (Hypertension)

• Initial dose: 0.1 mg twice daily (morning & bedtime)
  • Elderly patients: May require a lower initial dose.
• Maintenance dose: Increase by 0.1 mg/day at weekly intervals if needed.
  • Typical range: 0.2–0.6 mg/day (divided doses).
• Renal impairment: Adjust dosage & monitor closely.

Other Conditions

• ADHD:
  • Immediate-Release (IR): 5 mcg/kg/day
  • Extended-Release (ER): 0.1 mg/day for 8 weeks
• Menopausal Flushing: 0.1–0.4 mg daily
• Alcohol Withdrawal: 0.3–0.6 mg every 6 hours

🔹 Pediatric Use: Safety not well-established.

✔️ Drug Interactions

• CNS depressants (e.g., alcohol, barbiturates, sedatives) → Enhanced drowsiness
• Tricyclic Antidepressants (TCAs) → May reduce the hypotensive effect, requiring a dose increase
• Digitalis, β-blockers, Calcium Channel Blockers → Risk of bradycardia & A.V. block

✔️ 🚫 Do not use in patients with:

• Hypersensitivity to Clonidine

✔️ Side Effects

(Usually mild, improves with continued use)

• Common:
  • Dry mouth, dizziness, drowsiness, sedation
  • Constipation, nausea, diarrhea, abdominal pain
  • Irritability, ear pain

✔️ Pregnancy & Lactation

• Pregnancy: Category C → Avoid unless necessary
• Lactation: Passes into breast milk → Use caution

✔️ Precautions & Warnings

• Do NOT discontinue suddenly → Can cause:
  • Nervousness, agitation, headache, tremor, rapid BP rise
  • High catecholamine levels
• Discontinuation: Reduce dose gradually over 2-4 days
• If using with β-blockers: Withdraw β-blocker first, then taper Clonipres
