Acecard 1.25mg Tablet 1 Strip

✔️ Dosage & Administration

The dose should be individualized based on patient response and tolerability.

Hypertension:

• Initial dose: 1.25–2.5 mg once daily (in patients not on diuretics)
• Dose adjustment: Gradually at intervals of at least 2 weeks
• Maintenance dose: 2.5–20 mg daily (single or divided doses)
• A diuretic may be added if needed for better control

Heart Failure after Myocardial Infarction:

• Start within 2 days post-MI
• Initial dose: 2.5 mg twice daily
• If hypotension occurs: reduce to 1.25 mg twice daily
• Target dose: up to 5 mg twice daily

Prevention of Cardiovascular Events:

• Start: 2.5 mg once daily for 1 week
• Then: 5 mg daily for 3 weeks
• Maintenance: up to 10 mg once daily

Renal Impairment:

• Hypertension: Start with 1.25 mg once daily (max 5 mg/day)
• Heart failure: Start with 1.25 mg daily, may increase up to 2.5 mg twice daily based on tolerance

✔️ Side Effects

• Dizziness
• Headache
• Fatigue and weakness

Less common effects:

• Hypotension
• Dry cough
• Nausea, vomiting, diarrhea
• Skin rash or urticaria
• Reduced urine output

Rare but serious:

• Angioedema
• Allergic reactions
• Hyperkalemia

✔️ Drug Interactions

• Concomitant use with diuretics may cause significant hypotension
• Potassium-sparing diuretics may increase the risk of hyperkalemia
• May increase serum lithium levels when used together
• Alcohol may enhance blood pressure–lowering effects and impair alertness

✔️ Ramipril should not be used in:

• Patients with hypersensitivity to ramipril or other ACE inhibitors
• History of angioedema
• Concomitant use with sacubitril/valsartan (must allow washout period)
• Patients with renal artery stenosis (bilateral or single kidney)
• Hypotensive or hemodynamically unstable patients
• Use with aliskiren in diabetic or renal impairment patients
• Patients with diabetic nephropathy taking angiotensin receptor blockers
• Pregnancy

✔️ Pregnancy & Lactation

• Contraindicated during pregnancy due to risk of fetal harm
• Should be discontinued immediately if pregnancy occurs
• Not recommended during breastfeeding

✔️ Use cautiously in patients with:

• Renal impairment
• Liver disease
• Hyperkalemia
• Hypotension

✔️ Storage:

• Store below 30°C
• Protect from light and moisture
• Keep out of reach of children
• Do not use after the expiry date
