When taken after meals, the serum progesterone levels in the body reach their highest point. Progesterone has a high affinity for binding to blood proteins, with about 96-99% of it binding to these proteins, primarily serum albumin. In the liver, progesterone is converted to pregnanediol and pregnanolones. These metabolites are eliminated from the body in the form of glucuronide and sulfate conjugates through the bile and urine.

Dosage & Administration of
  • The recommended dose of progesterone is typically 100mg to 200mg per day, taken orally or vaginally in divided doses. However, the dosage may vary based on the condition being treated and the individual needs of the patient.
  • Progesterone can be used to maintain pregnancy in cases of threatened or recurrent abortion, with a recommended dosage of 200mg to 400mg per day in divided doses.
  • For in-vitro fertilization and embryo transfer, the recommended dosage is 200mg taken three times a day starting on the day of embryo transfer until pregnancy is confirmed. If pregnant, the dosage can be continued until the 12th week of pregnancy.
  • For hormone replacement therapy, the recommended dosage varies based on the regimen being used. In a sequential regimen, 200mg of progesterone is taken daily for the last two weeks of each therapeutic cycle. In a continuous regimen, 100mg of progesterone is taken daily throughout the month in conjunction with estrogen.
  • Progesterone can also be used in the oocyte donation program, with a recommended dosage of 100mg taken twice a day from the day of transfer until pregnancy is confirmed. The dosage may be increased to a maximum of 600mg per day and continued until the 12th week of pregnancy.
  • For luteal support, progesterone is taken at a dosage of 100mg three times a day starting on the 17th day of the cycle for 10 days in an induced cycle. If pregnant, the dosage is continued until the 12th week of pregnancy.
  • In cases of luteal phase insufficiency, the dosage of progesterone is gradually increased up to a maximum of 600mg per day in divided doses if required. For secondary amenorrhea, a dosage of 300mg of progesterone taken for 10 days can result in withdrawal bleeding in 80% of cases.
  • For premenstrual syndrome, the recommended dosage is 100-200mg of progesterone taken daily for 10 days from the 17th to the 26th day of each menstrual cycle. For benign mastopathy, a dosage of 200-300mg of progesterone is typically taken for 10 days per month, usually from the 17th to the 26th day of the monthly cycle.

Contraindications

  • Patients who are on warfarin, or oral anticoagulants, or have an increased risk of thromboembolic complications after surgery should also use caution and may require an increase in anticoagulant dose.
  • Women who use oral contraceptives may have an increased risk of breast and cervical cancer, as well as liver cancer. The risk of these cancers may increase with long-term use of oral contraceptives.
