Colimax 0.6mg Tablet is used to treat and prevent gout attacks and manage symptoms of FMF, a genetic disorder causing recurrent fevers and inflammation.

Colchicine reduces uric acid crystal buildup in joints, decreasing swelling and pain in gout. In FMF, it decreases amyloid protein production, alleviating pain in the abdomen, chest, or joints.

  • Inform your doctor of any allergies, blood disorders, heart, kidney, or liver disease.
  • Not recommended for patients with severe liver or kidney disease.
  • Avoid use during pregnancy for gout management; use caution in FMF if necessary.
  • Women of childbearing potential should use contraception during and for 3 months post-treatment.
  • Not recommended for children under 18 for gout; consult a doctor for FMF management in children under 5 and adolescents 10-18 years old.

Pharmacology

Colimax is FDA-approved to prevent and treat sudden severe gout attacks. In gout, uric acid crystals deposit in joints, prompting white blood cells to attempt to engulf them, releasing lactic acid and pro-inflammatory enzymes. This leads to the characteristic pain and swelling of gout. Colimax inhibits white blood cell migration to inflamed areas, reducing pain and inflammation.

An acute gout attack results from an inflammatory reaction to monosodium urate crystals deposited in joint tissue, exacerbated by more crystal accumulation. Leukocytes migrate to these sites, trying to engulf the crystals, releasing lactic acid and pro-inflammatory enzymes. This causes inflammation with severe pain, redness, and swelling. Colchicine inhibits the phagocytosis of uric acid by leukocytes and reduces lactic acid production, disrupting the cycle of urate crystal deposition and the inflammatory response, and alleviating the acute attack of gout.

Colchicine is orally absorbed, reaching a mean peak concentration (Cmax) of 2.5 ng/mL in 1 to 2 hours after a single dose on an empty stomach. The mean apparent volume of distribution is approximately 5 to 8 L/kg. Colchicine binds low to serum protein (39 ± 5%), primarily to albumin. It is metabolized by CYP3A4 into 2-O-demethylcolchicine and 3-O-demethylcolchicine. Plasma levels of these metabolites are minimal. Following multiple oral doses (0.6 mg twice daily), the mean elimination half-life is 26.6 to 31.2 hours.

Dosage & Administration of Colimax 0.6mg Tablet

Acute Gouty Arthritis:

  • First dose: 1 to 1.2 mg (two 0.5 mg or two 0.6 mg tablets) every 2 hours until pain and inflammation subside.
  • The total amount needed to control pain and inflammation during an attack typically ranges from 4 to 8 mg.
  • Pain and swelling usually reduce within 12 hours and disappear within 24 to 48 hours.
  • A three-day interval between courses is advised to minimize cumulative toxicity.

Prophylaxis During Intercritical Periods:

  • To reduce frequency and severity, colchicine may be administered continuously.
  • For patients with less than one attack per year, the usual dose is 0.5 or 0.6 mg per day, three or four days a week.
  • For more than one attack per year, the usual dose is 0.5 or 0.6 mg daily.
  • Severe cases may require two or three 0.5 mg or 0.6 mg tablets daily.

Prophylaxis Against Attacks in Patients Undergoing Surgery:

  • For patients with gout, even minor surgery can trigger an attack.
  • Administer one 0.5 mg or 0.6 mg tablet three times daily for three days before and three days after surgery.

Pediatric Use: Gout is rare in pediatric patients, and the safety and effectiveness of Colimax in this population have not been established.

Interaction

Co-administration with P-gp and/or CYP3A4 inhibitors (e.g., clarithromycin or cyclosporine) alters Colimax concentration.

Contraindications

Not recommended for patients with renal or hepatic impairment when used with P-gp or strong CYP3A4 inhibitors (e.g., clarithromycin or cyclosporine).

Pregnancy & Lactation
  • Pregnancy Category C: No adequate and well-controlled studies in pregnant women.
  • Excreted into human milk: Exercise caution when administered to nursing women.
Precautions & Warnings
  • Elderly: Use with caution due to increased sensitivity to side effects.
  • Gastrointestinal Disease: Exercise caution as the drug may exacerbate symptoms.
  • Cardiac, Renal, and Hepatic Impairment: Use with caution. The drug is not recommended for patients with severe impairment in these organs.
  • Prolonged Therapy: Not recommended due to potential cumulative toxicity.
  • Lactation: Colchicine enters breast milk. Use with caution during breastfeeding. The American Academy of Pediatrics Committee states that colchicine is "compatible" with nursing.
