Telfin 250mg Tablet is an antifungal medicine. It is used to treat fungal infections like athlete's foot (itchy, scaly, and white patches between the toes), jock itch (red, itchy, and ring-shaped rashes in the groin, the area where the upper thigh meets the lowest part of the stomach), ringworm (red, itchy, and circular-shaped rashes with clear skin in the middle affecting the skin, scalp, or nails), and nail infections. This medicine works by killing the fungi by damaging their cell membrane (protective covering) and making them weaker. Thus, it prevents the spread of fungal infections. 

Pharmacology

Terbinafine is classified as an allylamine antifungal medication that functions by inhibiting the biosynthesis of ergosterol, which is a crucial component of fungal cell membranes. This inhibition is caused by the suppression of the enzyme squalene epoxidase. As a result, the fungal cells die mainly because of the increased membrane permeability that results from the accumulation of high concentrations of squalene, but not because of ergosterol deficiency. The ability of terbinafine hydrochloride to kill the fungus depends on the concentration of the drug and the fungal species tested in vitro. Although the clinical significance of the in vitro data is unknown, terbinafine has been found to be effective against most strains of Trichophyton Mentagrophyte and Trichophyton rubrum, both in vitro and in clinical infections.

Dosage & Administration of Telfin 250mg Tablet

• The adult dose for oral onychomycosis is 250 mg once daily for 6 weeks (for fingernail infections) or 12 weeks (for toenail infections). For tinea pedis, tinea corporis, and tinea cruris, the recommended adult dose is 250 mg/day orally, either as a single dose or divided into two doses every 12 hours, for 2-4 weeks for tinea cruris, 2-6 weeks for tinea pedis, and 2-4 weeks for tinea corporis.
• In children with tinea capitis, the recommended dose varies based on weight. For those weighing less than 25 kg and over 4 years of age, the recommended dose is 125 mg/day orally for 6 weeks. For those weighing 25-35 kg, the recommended dose is 187.5 mg/day orally for 6 weeks. For those weighing over 35 kg, the recommended dose is 250 mg/day orally for 6 weeks.
• Terbinafine should not be used in patients with chronic or active liver disease. In patients with renal impairment, terbinafine should not be used if the creatinine clearance is less than 50 mL/min.

Contraindications
