• Quinidine, cimetidine, SSRIs, propafenone, and flecainide may elevate imipramine levels.

• Combining with alcohol can lead to drowsiness and impaired performance.
• Severe hypertension may occur with adrenaline, noradrenaline, or methylphenidate.
• Drugs such as guanethidine, bethanidine, debrisoquine, bretylium, methyldopa, and clonidine may lose effectiveness.
• This can occur with monoamine oxidase inhibitors (MAOIs). Administer with a gap of at least 3 weeks between treatments.

• Patients with any degree of heart block, cardiac arrhythmias, or recent myocardial infarction (MI) should not use imipramine.

• Imipramine's sedative effects are heightened with central nervous system depressants such as alcohol, sleeping medications, sedatives, or antihistamines.
• Imipramine may increase heart rate and stress the heart. It should be avoided or used with caution in patients with cardiovascular disease, especially those who have recently experienced a heart attack. Older adults or individuals with a history of heart disease may be at risk for arrhythmias, conduction abnormalities, heart failure, rapid heart rates, and stroke.
• Close monitoring is crucial during the initial stages of treatment or dose adjustments, particularly in younger populations (<24 years), as antidepressants may increase the risk of suicidal thoughts or behavior. This risk is also higher in cases of overdose or alcohol consumption.
• Starting imipramine therapy may trigger manic episodes or worsen symptoms of pre-existing psychotic conditions.
• Imipramine is excreted in breast milk. Nursing is not recommended as the effects on infants are uncertain but potentially concerning.
