✅ Description:
Doxophylline is a modern bronchodilator structurally distinct from theophylline due to the presence of a dioxolane group at position 7. It works primarily by selectively inhibiting phosphodiesterase-4 (PDE-4), resulting in relaxation of bronchial smooth muscles.
Unlike theophylline, it has minimal affinity for adenosine A1 and A2 receptors, which contributes to its improved safety profile. Additionally, it inhibits platelet-activating factor (PAF) and reduces leukotriene synthesis, further supporting its bronchodilatory effects.
Overdose may cause severe arrhythmias and seizures.

✔️ Dosage & Administration
- Elderly: 200 mg tablet, 2–3 times daily
- Adults: 400 mg tablet, 2–3 times daily or as directed by a physician
- Children:
- Above 12 years: 200 mg tablet or 10 ml syrup, 2–3 times daily
- 6–12 years: 6–9 mg/kg body weight twice daily
- Example: A 10 kg child may receive 3 ml (60 mg) twice daily
- Sustained-release (SR) formulation: 400 mg once daily if indicated

- Avoid concurrent use with other xanthine derivatives
- Limit intake of caffeine-containing foods and beverages
- Use cautiously with ephedrine or other sympathomimetic agents
- Certain drugs (e.g., erythromycin, lincomycin, clindamycin, allopurinol, cimetidine, propranolol, influenza vaccine) may reduce hepatic clearance, increasing plasma levels
- Phenytoin, anticonvulsants, and smoking may enhance drug clearance, potentially requiring dose adjustment
