Sitap (Sitagliptin)

Therapeutic Class: Dipeptidyl Peptidase-4 (DPP-4) Inhibitor

Sitagliptin selectively inhibits DPP-4, the enzyme responsible for inactivating incretin hormones.
Incretins such as GLP-1 and GIP stimulate glucose-dependent insulin release and reduce glucagon secretion.
By increasing incretin levels, Sitagliptin enhances insulin secretion and suppresses hepatic glucose output in a glucose-dependent manner.
Demonstrates high selectivity for DPP-4, with no inhibition of DPP-8 or DPP-9 at therapeutic doses.

Pediatric: Not established (<18 years).
Geriatric: Dose adjustment may be needed due to reduced renal function.
Renal Dose Adjustments:
Mild impairment (CrCl ≥50 mL/min): No adjustment.
Moderate impairment (CrCl 30–50 mL/min): 50 mg once daily.
Severe impairment / ESRD (CrCl <30 mL/min or on dialysis): 25 mg once daily.

Standard Dose: 100 mg once daily OR 50 mg twice daily.

May be taken with or without food.

No clinically significant interactions with metformin, glyburide, simvastatin, rosiglitazone, warfarin, or oral contraceptives.

Slight increase in digoxin levels (no dose adjustment usually required).

Doses up to 800 mg well tolerated; mild QTc prolongation observed.

Supportive measures recommended (gastric lavage, monitoring, ECG).

Sitagliptin is modestly dialyzable (13.5% removed in 3–4h session).

Known hypersensitivity to Sitagliptin (e.g., anaphylaxis, angioedema, Stevens–Johnson syndrome).

Side Effects

Pregnancy & Lactation

Pregnancy Category B. Use only if clearly needed.

Excreted in rat milk (unknown in humans); use with caution during lactation.
