Immediate-Release Tablets

The dosage should be tailored to each patient's needs, balancing effectiveness and tolerability. The maximum recommended dose is 2.5 mg linagliptin and 1000 mg metformin twice daily, taken with meals.

Recommended starting doses:

  • Patients not currently using metformin: 2.5 mg linagliptin + 500 mg metformin twice daily.
  • Patients already taking metformin: 2.5 mg linagliptin plus their current metformin dose twice daily.
  • Patients already using both medicines separately may switch to the combination tablet containing equivalent doses.
✔️ Dosage and Administration

Extended-release formulations should be taken once daily with a meal. The total daily dose should not exceed 5 mg linagliptin and 2000 mg metformin.

Recommended starting doses:

  • For patients new to metformin: 5 mg linagliptin + 1000 mg metformin XR once daily.
  • For patients already on metformin: 5 mg linagliptin combined with a similar daily metformin dose.
  • Patients switching from immediate-release formulations should receive an equivalent total daily dose.

Patients using the 2.5 mg/1000 mg extended-release strength should take two tablets together once daily.

✔️ Lino-M may interact with several medications, including:
  • Cationic drugs such as digoxin, quinidine, morphine, amiloride, trimethoprim, and vancomycin, which may compete with metformin for renal elimination.
  • Carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide, zonisamide) that can increase the risk of metabolic or lactic acidosis.
  • Strong P-glycoprotein or CYP3A4 inducers, such as rifampin, which may reduce the effectiveness of linagliptin.
  • Medications that raise blood sugar levels, including corticosteroids, diuretics, thyroid hormones, oral contraceptives, calcium channel blockers, phenytoin, and isoniazid.

Patients receiving these medications may require closer monitoring and dose adjustments.

Common side effects include:

  • Nasopharyngitis (common cold-like symptoms)
  • Diarrhea

The risk of hypoglycemia (low blood sugar) may increase when Lino-M is used together with sulfonylureas or other glucose-lowering medicines.

✔️ Low Blood Sugar (Hypoglycemia)

Although Lino-M alone has a relatively low risk of causing hypoglycemia, it can occur if you:

  • Skip meals.
  • Consume alcohol.
  • Exercise more than usual.
  • Use other diabetes medications simultaneously.
