✔️ Dosage & Administration

Immediate Release Tablets

• Maximum Dose: 2.5 mg Linagliptin + 1000 mg Metformin twice daily with meals.

Initial Dosing:

• Not on Metformin: 2.5 mg Linagliptin + 500 mg Metformin twice daily.
• Already on Metformin: 2.5 mg Linagliptin + current Metformin dose twice daily.

Extended Release Tablets

• Maximum Dose: 5 mg Linagliptin + 2000 mg Metformin once daily with meal.

Initial Dosing:

• Not on Metformin: 5 mg/1000 mg once daily with food.
• Already on Metformin: 5 mg Linagliptin + equivalent Metformin once daily.
• Switching from immediate release: Use the same total daily doses.

If using 2.5 mg Linagliptin + 1000 mg Metformin XR, take two tablets together once daily.

• Avoid alcohol: It increases the risk of low blood sugar.
• Be aware of medication interactions (including OTC meds like aspirin and cold remedies).
• Inform the family on how to respond to hypoglycemia.
• Traveling? Keep prescriptions, ID, and medical history.
• Consider wearing a diabetic ID bracelet or card.

✔️ Overdose Management

• Supportive care is key.
• Linagliptin is not dialyzable.
• Metformin is dialyzable; lactic acidosis is a major concern.
• Signs of overdose: severe weakness, vomiting, drowsiness, breathing problems.
