  6. Complicated urinary tract infections and acute pyelonephritis: 250 mg once daily for 7-10 days.
  7. Uncomplicated skin and soft-tissue infections: 500 mg once daily for 7-10 days.
  8. Complicated skin and soft-tissue infections: 750 mg once daily for 7-14 days.
  9. Enteric fever: 500 mg once daily for 7-14 days.
  10. Diarrhea, cholera, shigellosis & enteritis:
  • Mild to moderate case: 500 mg (single dose).
  • Moderate to severe case: 500 mg once daily for 3 days.

Children:

  1. Children 6 months to <5 years: 10 mg/kg every 12 hours.
  2. Children >5 years: 10 mg/kg every 24 hours.

The dosing of levofloxacin in patients with renal impairment is as follows:

For patients on hemodialysis or continuous ambulatory peritoneal dialysis (CAPD):

  • Initially, 500 mg daily, then 250 mg every 48 hours.
  • Alternatively, initially, 750 mg daily, then 500 mg every 48 hours.

For patients with a creatinine clearance (CrCl) of 20-49 ml/min:

  • Initially, 500 mg daily, then 250 mg every 24 hours.
  • Alternatively, initially, 750 mg daily, then 750 mg every 48 hours.

For patients with a CrCl of 10-19 ml/min:

  • Initially, 500 mg daily, then 250 mg every 48 hours.
  • Alternatively, initially, 750 mg daily, then 500 mg every 48 hours.
