Indications

• Fimoxyl 500 is prescribed for treating infections caused by susceptible ß-lactamase negative strains of microorganisms.
• The infections for which Amoxicillin is effective include ear, nose, and throat infections such as otitis media, sinusitis, tonsillitis, pharyngitis, and laryngitis.
• Fimoxyl 500 is also useful in treating lower respiratory tract infections, such as pneumonia, acute and chronic bronchitis, lung abscess, empyema, and bronchiectasis.
• Amoxicillin is effective in treating skin and soft tissue infections such as cellulitis, carbuncles, furunculosis, infected wounds, and abscesses.
• Fimoxyl 500 is useful for treating genitourinary tract infections such as pyelonephritis, cystitis, and urethritis.
• For the treatment of acute uncomplicated gonorrhoea, Amoxicillin is used as a treatment option.
• Short-term therapy using Amoxicillin is also indicated in cases of dental abscess.
• Amoxicillin, in combination with Clarithromycin and Lansoprazole (known as triple therapy), is used to treat patients with H. pylori infection and duodenal ulcer disease. Fimoxyl 500 also reduces the risk of recurrence of duodenal ulcer.

Pharmacology

Amoxicillin is a type of penicillin that can combat a diverse array of Gram-positive and Gram-negative bacteria due to its broad-spectrum nature. Its mechanism of action involves hindering the synthesis of cell wall. Fimoxyl 500 is capable of withstanding gastric acid and is quickly assimilated after being taken orally. Within 1 to 2 hours of consumption, the highest level of Amoxicillin in the bloodstream is achieved. The drug is distributed widely throughout the body's tissues and fluids in different concentrations.

Dosage

Adult: 250 mg three times daily, increasing up to 500 mg three times daily for severe infections.

Children (up to 10 years of age) : 125 mg three times daily, increasing up to 250 mg three times daily for severe infections.

Severe or recurrent purulent respiratory infection: 3 gm every 12 hours.

Otitis media: Recommended dose for Fimoxyl 500  is 1 g three times daily for adult and 40 mg/kg body weight daily in 3 divided doses for children (max. 3 g daily).

Pneumonia: Recommended dose is 500-1000 mg three times daily.

Dental abscess: Recommended dose is 3 gm, repeated after 10-12 hours.

Urinary tract infections: Recommended dose is 3 gm, repeated after 10-12 hours.

Gonorrhoea: Single dose of 2-3 gm with Probenecid 1 gm is recommended (Probenecid is contraindicated in children under 2 years).

In renal impairment: it may be necessary to reduce the total daily dosage.

Administration

To administer a reconstituted suspension, one must add the appropriate amount of the suspension to milk, fruit juice, or water. It is crucial to consume the mixture right away.

Interaction

• Concurrent usage of Amoxicillin and Probenecid can lead to heightened and extended blood levels of Amoxicillin.
• Fimoxyl 500 has the potential to impact gut flora, which may cause a reduction in estrogen reabsorption and a decrease in the effectiveness of combined oral estrogen/progesterone contraceptives.

Contraindications

Patients who are hypersensitive to penicillin must not take Fimoxyl 500 due to contraindications.

Pregnancy & Lactation

Fimoxyl 500 falls under the US FDA pregnancy category B, indicating that it is not expected to cause harm to a developing fetus, based on animal studies. However, there is a lack of sufficient and well-controlled studies in pregnant women. Therefore, the usage of this drug during pregnancy must be done only if it is absolutely necessary. Amoxicillin has been found to be present in human milk, and hence, administering this drug to a lactating mother requires careful consideration.
