• Bacterial eye/ear infections

✔️ Side Effects

• Common: Itching, redness, swelling, burning sensation, stinging, increased redness, lacrimation, and transient irritation.
• Serious: Bacterial and corneal ulcers, ocular burning and irritation, nonspecific conjunctivitis, conjunctival epithelial defects, and conjunctival hyperemia.

• Follow your doctor’s instructions for use precisely.
• Do not skip doses and complete the full course of treatment.
• Avoid touching the dropper tip to any surface to prevent contamination.
• Wait at least 5-10 minutes before applying other medications in the same eye/ear.
• Inform your doctor if symptoms do not improve within two days of starting treatment.
• Do not wear contact lenses while using this medication for an eye infection.

✔️ Pharmacology

Gentamicin is an aminoglycoside antibiotic that binds to the 30S subunit of bacterial ribosomes, disrupting protein synthesis and effectively killing bacteria. It is active against a wide range of Gram-positive and Gram-negative bacteria.

✔️ Dosage & Administration

• Eye:
  • Adults and Children: Instill 1-2 drops into the affected eye up to 6 times a day. For severe infections, initially instill 1-2 drops every 15-20 minutes, then reduce the frequency as the infection is controlled.
• Ear:
  • Adults and Children: Clean the area and instill 2-3 drops into the affected ear 3-4 times a day and at night, or more frequently if required.

Interactions

• Additive Effect with Neurotoxic and Nephrotoxic Drugs: Including cephalosporins, methicillin, amphotericin B, ciclosporin, cisplatin, potent diuretics (e.g., ethacrynic acid, furosemide).
• Potentiation of Anticoagulants: May enhance the effects of anticoagulants like warfarin and phenindione.
• Antagonistic Effects: May reduce the effectiveness of neostigmine and pyridostigmine.
• Increased Risk of Neuromuscular Blockade: With botulinum toxin.
• Indometacin: May increase plasma concentration of Gentamicin in neonates.

✔️ Contraindications

• Hypersensitivity to Gentamicin or any component of the product.
• Use is contraindicated in pregnancy and patients with myasthenia gravis.
• Not for injection. Do not use subconjunctivally or directly in the anterior chamber of the eye.

✔️ Pregnancy & Lactation

• Pregnancy: Category D. Gentamicin may cause fetal ototoxicity. The safety profile for eye/ear drops in pregnancy is not established; use only if essential.
• Lactation: Small amounts of Gentamicin have been detected in breast milk; breastfeeding should be discontinued during treatment.

✔️ Precautions & Warnings

• Use cautiously in patients with conditions predisposing them to ototoxicity and nephrotoxicity.
• Monitor plasma concentrations in patients receiving high doses or prolonged courses, especially in infants, the elderly, and patients with renal impairment, cystic fibrosis, or significant obesity.
• Monitor auditory and renal functions.
• Avoid contamination by not touching the tip of the dropper to any surface.
