✔️ Domperidone is a dopamine receptor blocker that acts mainly in the digestive system. It:

• Enhances stomach emptying
• Improves movement of food through the stomach and intestines
• Strengthens the lower esophageal sphincter
• Reduces reflux of stomach contents into the esophagus
• Helps control nausea and vomiting by acting on the chemoreceptor trigger zone (CTZ)

Because it crosses the blood-brain barrier poorly, it causes fewer neurological side effects than many other dopamine antagonists.

✔️ Use caution with:

• Anticholinergic medicines, which may reduce the effectiveness of domperidone
• Antacids and acid-suppressing medications, which can decrease its absorption if taken simultaneously
• Strong CYP3A4 inhibitors that can increase domperidone levels, including:
  • Azole antifungals
  • Macrolide antibiotics
  • HIV protease inhibitors
  • Nefazodone

Domperidone may help reduce nausea and digestive side effects caused by dopamine agonists such as:

• Levodopa
• Bromocriptine

✔️ Dosage & Administration

Take Don-A 15–30 minutes before meals, and if needed, before bedtime.

Adults

• Usual dose: 10–20 mg every 6–8 hours
• Maximum daily dose: 80 mg

Children

• 0.2–0.4 mg/kg every 6–8 hours
• Dosing may vary according to body weight and formulation

Acute nausea and vomiting

• Adults: 20 mg every 6–8 hours
• Treatment should generally not exceed 12 weeks

✔️ Don-A should not be used in patients with:

• Allergy to domperidone
• Newborn infants
• Gastrointestinal bleeding
• Intestinal obstruction or perforation
• Prolactin-secreting pituitary tumors (prolactinoma)

✔️ Side Effects

• Mild abdominal cramps
• Skin rash
• Hives (urticaria)

Rare side effects:

• Increased prolactin levels
• Breast enlargement in males (gynecomastia)
• Milk production unrelated to breastfeeding (galactorrhea)
• Extrapyramidal reactions (abnormal movements), especially in young children

✔️ Pregnancy and Breastfeeding

• Use during pregnancy only when the expected benefit outweighs potential risks.
• Domperidone passes into breast milk in small amounts.
• Breastfeeding is generally not recommended during treatment unless advised by a doctor.

✔️ Overdose

Symptoms may include:

• Drowsiness
• Confusion or disorientation
• Abnormal involuntary movements, particularly in children

Management typically involves:

• Activated charcoal
• Close medical monitoring
• Symptomatic treatment as needed

✔️ Use in Special Populations

Infants

• Extra caution is required because their blood-brain barrier is not fully developed, increasing the risk of neurological side effects.

Liver Disease

• Use carefully because domperidone is extensively metabolized by the liver.

Kidney Disease

• In severe renal impairment, dosing frequency may need to be reduced due to prolonged drug elimination.
