It works by blocking the action of dopamine, a chemical messenger in the brain that affects thoughts and mood.

How to Administer Fenazine 25mg Injection?

Fenazine 25mg Injection will be given to you only by a doctor or a nurse into the muscle (intramuscularly) or under the skin (subcutaneously). Your doctor will decide the correct dose and duration for you depending on your age, body weight, and disease condition.

Side Effects of Fenazine 25mg Injection

• Common: Anticholinergic effects, sedation, weight gain, erectile dysfunction, oligomenorrhea or amenorrhea.
• Less Common: Orthostatic hypotension (after IM injection), tachycardia, agitation, anxiety, cerebral edema, depression, dizziness, euphoria, headache, poikilothermia, restlessness, weakness, anorexia, constipation, dyspepsia, ileus, lens opacities (with prolonged use).
• Uncommon: ECG changes, photosensitivity, pruritus, diarrhea, blood dyscrasia, galactorrhea, ejaculatory disorder.
• Rare: Seizure, priapism, cholestatic jaundice.

Quick Suggestions:

• Use with caution in pregnant women.
• Not recommended during breastfeeding.
• Not recommended for children below 12 years.
• Use with caution in elderly patients, especially those with dementia-related psychosis, due to an increased risk of dementia.

Indications:

Fluphenazine is used in the treatment of psychotic symptoms such as hallucinations, delusions, and agitation.

Pharmacology

• Fluphenazine blocks postsynaptic dopamine D1 and D2 receptors in the mesolimbic system.
• Decreases the release of hypothalamic and hypophyseal hormones.
• Reduces aggressiveness with the disappearance of hallucinations and delusions.

Dosage & Administration of Fenazine 25mg Injection

Adult Dose:

• Indication: Psychoses; Mania; Schizophrenia
• Formulation: Intramuscular (as decanoate)
• Initial Dose: 12.5 mg
• Adjustment: Based on individual response
• Maintenance Dose: 12.5-100 mg at intervals of 2-6 weeks
• Increment for Doses >50 mg: Incremental steps of 12.5 mg, made slowly
• Elderly: 6.25 mg adjusted according to response
• Hepatic Impairment: Use with caution; monitor

Child Dose:

• Safety and Efficacy: Not established

Renal Dose:

• Renal Impairment: Use with caution; monitor

Interaction

• Reduced Antihypertensive Effects: Reduces antihypertensive effects of guanethidine, methyldopa, and clonidine.
• Lithium Toxicity: Lithium toxicity.
• Reduced Bioavailability: Reduced bioavailability with antacids.
• Increased Risk of Arrhythmia: Increased risk of arrhythmia when used with drugs that prolong QT interval.
• Electrolyte Disturbance: This may cause electrolyte disturbance when used with diuretics.
• Potentially Fatal: Additive CNS depressant effects with alcohol, barbiturates, hypnotics, sedatives, opiates, and antihistamines.

Contraindications

• Hypersensitivity: Fluphenazine Decanoate oily Injection is contraindicated in patients with hypersensitivity to the drug.
• Depressed States: Contraindicated in comatose or severely depressed states.
• Blood Dyscrasias: Contraindicated in the presence of blood dyscrasias.
• Liver Disease: Contraindicated in patients with liver disease.
• Pregnancy (3rd Trimester): Contraindicated in the third trimester of pregnancy.
• Lactation: Contraindicated during lactation.

Pregnancy & Lactation

The safety for the use of Fluphenazine decanoate during pregnancy has not yet been established; therefore the possible hazards should be weighed against the potential benefits when administering this drug to pregnant patients.

Precautions & Warnings

• Special precautions in convulsive disorders, hepatic, renal, cerebrovascular, respiratory, and cardiovascular diseases.
• Precaution in phenochromocytoma, bone marrow suppression, elderly or debilitated patients.
• Lactation: Drug enters breast milk; not recommended.
