Dosage & Administration of Voltalin Suppository 25mg

Diclofenac Suppository: For adults: 50 mg suppository 2-3 times daily. The maximum daily dose is 150 mg.

Interaction

• Methotrexate: Diclofenac may increase serum levels of methotrexate, potentially leading to methotrexate toxicity. Close monitoring of methotrexate levels and adjustment of dosage may be necessary.
• NSAIDs and Anticoagulants (e.g., warfarin): Concomitant use of diclofenac with other NSAIDs or anticoagulants like warfarin increases the risk of gastrointestinal bleeding. Caution is advised, and patients should be monitored for signs of bleeding.
• Ciclosporin or Triamterene: Diclofenac may increase the risk of nephrotoxicity when used concurrently with ciclosporin or triamterene. Renal function should be closely monitored in patients receiving this combination.
• Ophthalmic Preparations Containing Corticosteroids: Concurrent use of diclofenac with ophthalmic preparations containing corticosteroids may increase the risk of corneal complications, particularly in patients with pre-existing corneal inflammation. Caution is advised, and patients should be monitored for adverse effects.
• Colestyramine and Colestipol: These agents may reduce the bioavailability of diclofenac. Administration of diclofenac should be separated from these bile acid sequestrants to avoid decreased efficacy.
• Sucralfate: Administration of diclofenac after sucralfate may decrease its plasma concentration. To avoid reduced efficacy, diclofenac should be administered at least two hours before or after sucralfate.
• Ophthalmic Acetylcholine and Carbachol: Topical application of diclofenac may reduce the efficacy of ophthalmic acetylcholine and carbachol. Caution is advised when using these medications concomitantly.
• Lithium and Digoxin: Diclofenac may increase serum levels of lithium and digoxin. Monitoring of lithium and digoxin levels and adjustment of dosage may be necessary when used concomitantly with diclofenac.

Contraindications

• In patients with active or suspected peptic ulcer or gastrointestinal bleeding.
• Contraindicated for patients in whom attacks of asthma, urticaria, or acute rhinitis are precipitated by aspirin or other NSAIDs.
• Contraindicated for patients hypersensitive to local anesthetics of the amide-type.

Pregnancy & Lactation

• Diclofenac should be used with caution during pregnancy, particularly avoiding the first trimester.
• The use of diclofenac gel during pregnancy is not recommended due to limited clinical data.
• Diclofenac is excreted in breast milk but is generally considered safe for breastfeeding.

Precautions & Warnings

• Monitoring for peptic ulcers and gastrointestinal bleeding is recommended, especially in elderly patients.
• Avoid contact with eyes or mucous membranes when using diclofenac gel.
