Indications of Erian
Pharmacology
The local anaesthetic Cinchocaine Hydrochloride prevents or relieves the severe pain associated with strangulated haemorrhoids, fissures, and perianal haematomata, whereas the corticosteroid (Hydrocortisone) acts as a decongestant, anti-inflammatory, and anti-pruritic agent, reducing itching, inflammation, and mucous discharge. Most infections that may already be present or emerge in lesions of the anorectal area will be eradicated by the broad spectrum antibiotic Framycetin Sulphate, and Esculin has a skin protecting function.
Dosage and Administration
Ointment: Apply the ointment in a small quantity with the finger, on the painful or pruritic area, morning and evening and after each stool. For deep application, insert the cannula or applicator to full extent and squeeze tube gently from lower end whilst withdrawing.
Suppository: A suppository is inserted morning and evening, and after each stool.
Contraindications
Any of the four substances has a history of causing hypersensitivity. Dermal application of corticosteroids has been found to be teratogenic in animals. Because these chemicals are absorbed through the skin, teratogenicity from topical treatment cannot be ruled out. As a result, this product should not be used by women who are pregnant. In the treatment of herpes simplex, vaccinia or varicella, or tuberculous infection of the anal region, topical corticosteroid formulations are contraindicated.
Side Effects of Erian
Long-term continuous topical corticosteroid treatment should be avoided as much as possible because it can produce atrophic changes in the skin, such as thinning, loss of elasticity, dilation of superficial blood vessels, telangiectasia, and ecchymoses. When occlusive dressings are applied, these alterations are more likely to occur. When high amounts of corticosteroids are utilized or when they are administered to vast portions of the body or to damaged skin; when strong topical corticosteroids are used; and when the occlusive dressing technique is used, systemic absorption of topically applied corticosteroids may occur. Hypothalamic-pituitary-adrenal axis depression
Pituitary-adrenal axis with consequent suppression of the adrenal gland may occur. These effects are most likely to be severe in children. Growth may be retarded and a Cushingoid state may be produced. Benign intracranial hypertension has been rarely reported.
Pregnancy & Lactation
The safety of using topical corticosteroids during pregnancy has yet to be determined. As a result, they should not be used on wide areas, in big volumes, or for lengthy periods of time during pregnancy.
Precautions & Warnings
If sensitization occurs, stop using the product. Other specialized anti-infection, anti-allergy, and anti-cause efforts must not be overlooked. The likelihood of systemic corticosteroid effects from chronic administration of this preparation, however remote, should be considered. Patients should be recommended to tell their future doctors about their previous use of hydrocortisone.
