Secnidazole, a second-generation 5-nitroimidazole derivative, is rapidly absorbed after oral administration. Following a 2 g oral dose, peak plasma concentration is achieved within 3 hours. It has a long elimination half-life of ~20 hours, ensuring therapeutic serum levels for up to 72 hours from a single dose. Around 50% of the ingested dose is excreted in urine within 120 hours. Its long half-life provides sustained antiprotozoal activity.
Route: Oral (preferably just before meals).
Acute Intestinal Amoebiasis
Adults: 2 g single dose
Children: 30 mg/kg single dose
Asymptomatic Amoebiasis (minute & cystic form)
Adults: 2 g once daily for 3 days
Children: 30 mg/kg once daily for 3 days
Hepatic Amoebiasis
Adults: 1.5 g/day (single or divided dose) for 5 days
Children: 30 mg/kg/day (single or divided dose) for 5 days
Note: Drainage of abscess should be performed if suppuration is present
Giardiasis
Adults: 2 g single dose
Children: 35–50 mg/kg single dose
Trichomoniasis
Adults: 2 g single dose (both partners must be treated simultaneously)
Contraindicated with Disulfiram – may cause confusion and paranoid reactions.
Caution with Warfarin – may potentiate anticoagulant effect, increasing bleeding risk.
Known hypersensitivity to Secnidazole or other imidazole derivatives.
Common: Nausea, metallic taste, epigastric pain, glossitis, stomatitis.
Occasional: Urticaria, reversible moderate leukopenia.
Rare: Vertigo, paresthesia, peripheral neuropathy, motor incoordination.
GI intolerance (nausea, vomiting, abdominal pain) has been reported in very rare cases.
Pregnancy: Should be avoided in the first trimester. May be prescribed in later trimesters if necessary.
Lactation: Contraindicated, as the drug passes into breast milk.
Avoid alcohol consumption during therapy (risk of disulfiram-like reaction).
Use cautiously in patients with a history of blood dyscrasias.
Store in a cool, dry place, protected from heat and moisture. Keep out of reach of children.
