Linzo is an antibiotic used to treat serious infections caused by certain Gram-positive bacteria, including:
Blocking bacterial protein synthesis
Binding to the 23S rRNA of the 50S ribosomal subunit
Preventing formation of the 70S initiation complex needed for bacterial growth
This action stops bacteria from multiplying and helps control the infection.
Oral tablets: To be taken by mouth, with or without food.
Intravenous (IV) infusion: Administered by IV infusion over 30 to 120 minutes.

Neonates (<7 days):
Preterm neonates (<34 weeks gestational age) may have reduced clearance.
Initial dose: 10 mg/kg every 12 hours.
If clinical response is inadequate, dosing every 8 hours may be considered.
By 7 days of age, all neonates should receive 10 mg/kg every 8 hours.
No dosage adjustment is required when switching from IV to oral administration.
Inspect IV solution for particulate matter before use.
A yellow discoloration may occur and does not affect potency.
Do not use IV containers in series connections.
Do not add other drugs or additives to the solution.
Discard any unused portion after single use.
No dose adjustment is needed in mild to moderate hepatic impairment.
Pharmacokinetics in severe hepatic impairment have not been established.
Pediatric dosing is as described above.
Linezolid is not recommended for empiric treatment of CNS infections in children.

Hematologic monitoring:
Complete blood counts (CBC) should be monitored weekly, especially in patients receiving Linzo for more than two weeks, those with pre-existing myelosuppression, patients taking concomitant bone marrow–suppressive drugs, or individuals with chronic infections who have received prior or concurrent antibiotic therapy.
Visual disturbances:
Patients experiencing symptoms such as reduced visual acuity, changes in color vision, blurred vision, or visual field defects should undergo immediate ophthalmologic evaluation.
Restricted indications:
Linzo is not approved for the treatment of catheter-related bloodstream infections or catheter-site infections.
Clostridium difficile–associated diarrhea (CDAD):
CDAD has been reported up to two months after antibiotic therapy. If CDAD is suspected or confirmed, ongoing antibacterial therapy not directed against C. difficile may need to be discontinued.
Blood pressure and cardiovascular risk:
Linzo should be used with caution in patients with uncontrolled hypertension, pheochromocytoma, or thyrotoxicosis, and in those receiving sympathomimetic, vasopressor, or dopaminergic agents, unless blood pressure is closely monitored.
Metabolic disturbances:
Patients who develop recurrent nausea, vomiting, unexplained metabolic acidosis, or low bicarbonate levels during treatment should receive prompt medical assessment.
Hypoglycemia:
If hypoglycemia occurs, dose adjustment or discontinuation of insulin, oral hypoglycemic agents, or Linzo may be required.
Store Linzo formulations at room temperature (15°C–30°C).
