Olmetab Plus 20mg/12.5mg Tablet

✔️ Overdose Effects

• Olmesartan: May cause hypotension and tachycardia. Supportive care is recommended.
• Hydrochlorothiazide: Overdose may result in dehydration and electrolyte imbalance, including hypokalemia and hypochloremia, which can increase the risk of cardiac arrhythmias.

✔️ Side Effects

Common adverse effects may include:

• Headache
• Dizziness
• Nausea
• Hyperuricemia
• Upper respiratory tract infection
• Urinary tract infection

Other possible effects include:

• Chest pain
• Back pain
• Peripheral edema
• Abdominal discomfort
• Dyspepsia
• Gastroenteritis
• Diarrhea

✔️ Olmetab Plus should not be used in patients with:

• Hypersensitivity to Olmesartan, Hydrochlorothiazide, or any component of the formulation
• Anuria
• Allergy to sulfonamide-derived medications

✔️ Drug Interaction

Olmesartan:
No major interactions have been observed with hydrochlorothiazide, digoxin, or warfarin. Since Olmesartan is not metabolized by the cytochrome P450 enzyme system, significant CYP450-related interactions are unlikely.

Hydrochlorothiazide:
Caution is advised when used with:

• Alcohol, barbiturates, or narcotics due to increased risk of orthostatic hypotension
• Antidiabetic medications, as dose adjustments may be required
• Other antihypertensive drugs because of the additive blood pressure-lowering effects
• Corticosteroids or ACTH
• Lithium, due to the increased risk of toxicity

✔️ Dosage & Administration

The usual starting dose is one tablet containing 20 mg Olmesartan and 12.5 mg Hydrochlorothiazide once daily. Dosage may be adjusted according to the patient’s blood pressure response, typically every 2–4 weeks, up to a maximum of two tablets of 40/25 mg once daily, as directed by a physician.

• Renal Impairment: Standard dosing may be used if creatinine clearance is above 30 mL/min. It is not recommended in severe renal impairment, where loop diuretics are generally preferred.
• Hepatic Impairment: No dosage adjustment is usually required.
• Pediatric Use: Safety and efficacy have not been established in children.
• Geriatric Use: Elderly patients may require cautious dose selection.

✔️ Precautions & Warnings

• Periodic monitoring of serum electrolytes is recommended to detect conditions such as hypokalemia, hyponatremia, or hypochloremic alkalosis.
• Hyperuricemia may occur during therapy.
• Use cautiously in patients with impaired renal function.

✔️ Pregnancy & Lactation

The safety of Olmetab Plus during pregnancy and breastfeeding has not been established. Use during these conditions is not recommended, and the medication should be discontinued if pregnancy occurs.
