Propranolol Hydrochloride is a non-selective β-adrenergic blocker that reduces heart rate, myocardial contractility, and oxygen demand by antagonizing catecholamines at β-receptors. It is well absorbed from the GI tract but undergoes first-pass hepatic metabolism. Peak plasma levels occur within 1–1.5 hours. Elimination half-life: 3–6 hours. Metabolites are excreted via urine.

✔️ Dosage & Administration:

Adults:

Hypertension: Start 80 mg twice daily; maintenance 160–320 mg/day.
Angina: 40 mg 2–3 times daily; maintenance 120–240 mg/day.
Thyrotoxicosis: 10–40 mg, 3–4 times daily.
Anxiety: 40 mg once daily, may increase to TDS.
Migraine prophylaxis: 40 mg 2–3 times daily; maintenance 80–160 mg/day.

Children (Hypertension):

Neonates: 0.25–0.5 mg/kg TDS (adjust as needed).
1 month–12 years: 0.25–1 mg/kg TDS (max 5 mg/kg/day).
12–18 years: Start 80 mg twice daily; maintenance 160–320 mg/day.

Cold extremities, nausea, dizziness, fatigue, insomnia, paresthesia.

✔️ Drug Interactions:

Caution with verapamil, salbutamol, levosalbutamol, ergot alkaloids, lidocaine, and phenobarbital.

✔️ Contraindications:

Bronchospasm, cardiogenic shock, AV block (2nd/3rd degree), uncontrolled diabetes mellitus.

✔️ Pregnancy & Lactation

Category C: avoid in pregnancy unless necessary.

Excreted in breast milk; use with caution during lactation.

Caution in hepatic/renal impairment, pregnancy, and breastfeeding.
