✔️ Uses of Epleron 25

• Hypertension

✔️ Dosage & Administration of Epleron 25

For Congestive Heart Failure (Post-Myocardial Infarction):

• Initial dose: 25 mg once daily
• Target dose: 50 mg once daily (achieved within 4 weeks if tolerated)
• Can be taken with or without food

For Hypertension:

• Can be used alone or with other antihypertensive agents
• Starting dose: 50 mg once daily
• If blood pressure remains uncontrolled: Increase to 50 mg twice daily
• Higher doses (above 100 mg daily) are not recommended due to:
  • Lack of additional blood pressure benefits
  • Increased risk of hyperkalemia (high potassium levels)

Use in Special Populations:

• Pediatric patients: Safety and efficacy not established
• Geriatric patients: No significant differences in safety or effectiveness were observed

Note: This medication should be taken as per a physician’s recommendation.

✔️ Side Effects

Common side effects include:

• Headache, dizziness
• Gastrointestinal issues (diarrhea, stomach pain, nausea)
• Cough or flu-like symptoms

Serious allergic reactions (rare):

• Rash, itching, swelling
• Severe dizziness
• Difficulty breathing

✔️ Drug Interactions

• Increased risk of hyperkalemia when taken with:
  • ACE inhibitors & angiotensin receptor blockers (ARBs)
  • Ciclosporin, Tacrolimus, Trimethoprim
• Reduced antihypertensive effect when taken with:
  • NSAIDs, glucocorticoids, tetracosactide
• Enhanced hypotensive (blood pressure-lowering) effect when taken with:
  • Alpha-1 blockers (e.g., Alfuzosin, Prazosin)
  • Tricyclic antidepressants (TCAs), Amifostine, Baclofen, Neuroleptics
• Increased plasma levels when taken with:
  • Mild to moderate CYP3A4 inhibitors (e.g., Fluconazole, Erythromycin, Saquinavir, Amiodarone, Diltiazem, Verapamil)

✔️ Epleron should not be used in patients with:

• Hyperkalemia (high potassium levels)
• Severe renal impairment (Creatinine clearance <30 ml/min)
• Severe hepatic impairment
• Concomitant use with potent CYP3A4 inhibitors, such as:
  • Ketoconazole, Itraconazole, Nefazodone, Troleandomycin, Clarithromycin, Ritonavir, Nelfinavir
• Use with other potassium-sparing diuretics

✔️ Pregnancy & Lactation

• Pregnancy:
  • There are no well-controlled studies in pregnant women.
  • Should only be used if the benefits outweigh the potential risks to the fetus.
• Lactation:
  • It is unknown whether Eplerenone is excreted in human milk.
  • A decision must be made to either discontinue breastfeeding or discontinue the drug, considering its importance to the mother.

✔️ Use with caution in patients with:

• Hyperkalemia
• Severe kidney disease
• Diabetic patients with CHF post-MI, especially those with proteinuria (protein in urine)
• Monitor potassium levels regularly to avoid complications.

✔️ Storage Conditions

• Store at temperatures below 30°C, in a cool and dry place.
• Keep away from light and moisture.
• Keep out of reach of children.
